We’re seeking a caring and dependable PRN Hospice RN Field Nurse to join our team and provide quality, individualized care to patients and families throughout our service area.

Position Summary

The PRN Hospice RN Field Nurse provides professional nursing care to hospice patients as prescribed by the physician and in accordance with the State Nurse Practice Act and agency policies. This position delivers care in both home and facility settings on an as-needed basis and supports the interdisciplinary team in maintaining quality and compliance.

Key Responsibilities

• Provide skilled nursing care for assigned hospice patients, ensuring their physical, emotional, and spiritual needs are met.

• Complete admissions, routine visits, evaluations, imminent visits, and PRN or on-call visits as scheduled.

• Develop and update individualized care plans based on comprehensive assessments.

• Collaborate with the interdisciplinary team to review cases, manage symptoms, and ensure coordinated care.

• Maintain timely, accurate, and complete documentation that meets agency and Medicare standards.

• Communicate promptly with physicians, team members, patients, and families regarding changes in condition.

• Provide patient and family education, emotional support, and end-of-life counseling as needed.

• Uphold agency policies, ethical standards, and all applicable laws governing hospice care.

Qualifications

• Active Registered Nurse (RN) license in the State of Oklahoma.

• Minimum 2 years of hospice experience preferred.

• Recent direct patient care experience in hospice or institutional settings preferred.

• Excellent verbal, written, and electronic communication skills.

• Strong decision-making, time management, and critical-thinking abilities.

• Valid driver’s license, reliable transportation, and active auto liability insurance.

• Willingness to travel within the service area under various weather conditions.

Functional & Physical Requirements

• Ability to lift up to 50 lbs. and assist adult clients.

• Good color vision and hearing acuity for accurate patient assessment.

• Comfortable working independently in home and facility environments.

• Adaptable and able to manage changing priorities in a dynamic setting.

Schedule & Compensation

• PRN (as-needed) position – flexible hours and scheduling.

• Compensation based on visit type and experience.

• Mileage reimbursement available.
