SUPERVIOR: SSN/CHR Coordinator
FLSA: Non-Exempt
SALARY: $13.00-$18.00
ALL FULL TIME POSITIONS: In addition to your pay, your salary also includes Medical, Dental and Vision Insurance. The Biweekly deduction from employee paycheck is $40.00 for Employee Only, $95.00 Employee plus Children, $120.00 Employee plus Spouse, and $140.00 Employee plus family.
CPN Health Services personnel may be assigned to provide services to patients of all ages including but not limited to Indian Children and will require a criminal history background screening.
JOB SUMMARY: Serves as an assistant to the SSN/CHR coordinator and to the CHR program. Recovers numbers from RPMS for reports, answers phones and return calls as directed to coordinate CHR visits and/or other services, etc.,, work as directed with external/internal agencies for needed Elder services, RPMS documentation for transports and other nonmedical entries, maintain CPR renewals, employee requirements for learning, and licensure. Deliver medications and/or supplies to CHR patients, provides transport when needed, and manage the transport calendar when requests for transport are received. Assist with Lending Closet inventory and keeping records up to date. Provides information about available Elder services and health programs. Assist with planning community health promotion and disease prevention events.
ESSENTIAL FUNCTIONS:
- Serves as an assistant and support person to CHR program and to SSN/CHR Coordinator
- Assists with collecting data and maintaining required records and reports
- Maintain Lending Closet data and supplies
- Patient advocacy
- Medication delivery and transports as needed
- Documentation of nonmedical entries into RPMS
ADDITIONAL DUTIES:
- Perform other duties and projects as required by management
- Assists in planning and participating in community health promotion and disease prevention events
KNOWLEDGE, SKILLS, AND ABILITIES:
- Ability to communicate effectively and accurately with patients and staff.
- Ability to maintain strict confidentiality regarding all patients encounters.
- Computer skills
- Specialized knowledge, skills, and abilities to successfully perform duties of
the position.
MINIMUM QUALIFICATIONS FOR CONSIDERATION:
- Certified in Basic Life Support (CPR) or becomes certified within 3 months of employment.
- Must have a valid driver’s license and driving record that allows access to CPN vehicles for daily work function.
- Computer skills required for documentation in the patient record and retrieving data for reports.
PHYSICAL REQUIREMENTS:
- Sitting and standing for prolonged periods of time.
- Some lifting, bending, stretching, and kneeling may be required.
ENVIRONMENTAL CONDITIONS:
- General clinical and home environment with possible exposure to chemicals, blood borne pathogens and patient body fluids.
