Job Summary:
The Physical Therapist is responsible for the planning, provision, and coordination of all aspects of direct patient care in accordance with established clinic and departmental policies and procedures, maintaining the highest degree of quality care in a safe environment. This role collaborates with other members of the patient care team to uphold the highest standards of professional physical therapy practice.
The Physical Therapist exercises independent judgment, assumes responsibility and accountability for the delivery of therapy services provided by Physical Therapist Assistants, and participates in the orientation and mentoring of new staff.
The incumbent is expected to uphold the highest standards of ethical and moral conduct, act in the best interest of Three Crosses Regional Hospital, and fully support the organization’s mission, vision, and values.
Primary Responsibilities
- Provides high quality patient care and assessment in accordance with stablished practice standards and policies in a safe environment.
- Reviews patients’ medical history, referrals and physician notes from the patient charts or other medical records.
- Establishes a plan of care to improve patients’ functions and movement.
- Administers treatment programs for patients not limited to therapeutic exercises and activities, application of physical modalities i.e. heat, cold and light.
- Evaluate and record patient’s progress, modify plan of care and discuss with physical therapy assistants an established plan of care and treatment procedures.
- Educate patients and their families on recovery process and train families/caregivers on how to perform patient care.
- Performs therapeutic procedures by administering exercises and instructing, encouraging, and assisting patients in performing physical activities, ambulatory functional activities, and activities of daily living.
- Determine appropriate assistive devices such as crutches, canes and prostheses.
- Instructs and counsels patients by describing therapeutic regimens, giving normal growth and development information, and promoting wellness and health maintenance.
- Demonstrates appropriate knowledge and correct application of clinical techniques, procedures and programs for clinical setting and patient populations served.
- Plans and make appropriate discharge plans.
- Assesses the risk for safety and implements appropriate precautions.
- Collects data required to complete research projects, clinical outcomes studies, and or department quality improvement projects in compliance with Three Crosses and joint commission standards.
- Serves as a clinical resource for other health care professionals.
Education and Experience
- Required: Graduate from an accredited physical therapy program.
- Required: At least 1 year of related experience
- Required: Meet all Employee Health Requirement
- Required: Current Physical Therapy license, registration and or certification as per state regulations.
- Required: BLS certification
