Position Overview & General Responsibilities:
The primary responsibility of the Assisted Living/Memory Support Clinical Coordinator (AL/MS CC) is to oversee and coordinate the delivery of high-quality nursing care within designated units. This role includes providing leadership, technical guidance, instruction, and support to nursing staff. We are seeking a dedicated Clinical Coordinator to lead and support our Assisted Living & Memory Support neighborhoods. This role is responsible for ensuring high-quality resident care, strong team performance, and compliance with clinical and administrative standards. Additionally, the AL/MS Nurse Manager will assume the responsibilities of the Clinical Nursing Supervisor in their absence.
Key Responsibilities
- Oversee daily operations of Assisted Living and Memory Support neighborhoods
- Supervise and support team members within Memory Support neighborhood
- Collaborate with and serve as a clinical resource to the Clinical Nursing Supervisor
- Process weekly payroll and track attendance in coordination with HR
- Manage hiring, onboarding, and training for new team members
- Address performance issues and coordinate disciplinary actions as needed. Conduct staff performance reviews (90-day, annual), manage merit increases
- Comfort collaborating amongst interdisciplinary teams to offer appropriate support and efficient and impactful training
- Participate in on-call rotation and work varied shifts to support all teams
- Oversee treatment for residents including assessments, transcribing orders, and move-in/move-out support
- Ensure timely communication with physicians regarding resident condition changes
- Complete ISPs, UAIs, fall risk, skin integrity, and other required assessments
- Lead or participate in service plan meetings and quality assurance processes
- Maintain accurate documentation and infection/fall tracking reports
- Conduct and document team member education quarterly
- Coordinate medical record requests and insurance documentation
- Review, update, and help develop policies and procedures as assigned
- Support a safe, organized, and regulatory-compliant environment
- Serve as a role model for professional conduct, communication, and resident-centered care
- Assist Infection Control Nurse when necessary for vaccinations
- Other duties as assigned
Requirements
- Registered Nurse or Licensed Practical Nurse in Virginia or transferrable license to Virginia.
- Education and Experience equivalent to an associate’s degree preferred.
- Minimum of 2-5 years of nursing experience; preferably in long term care.
- Must have or be willing to obtain CPR certification.
- Supervisory skills required; experienced preferred.
- RAI/ISP/UAI experience preferred.
- Ability to work with confidential information and maintain its integrity.
- Strong customer service skills; maintain a hospitality approach to engaging with all individuals.
