POSITION: Health Educator
RESPONSIBLE TO: HIV Program Manager
FLSA Status: Non-Exempt
Position Summary
Under the supervision of the HIV Program Manager, The Health Educator works closely with the HIV Services Department to ensure the development and expansion of the PrEP patient navigation and adherence-counseling program at Imperial Beach Community Clinic (IBCC). The Health Educator will work directly with patients in the community, as well as internally at IBCC to encourage access to and coverage for PrEP medications. The Health Educator will ensure that participating clients receive relevant HIV/STI-related education and services, sexual health information, and navigation to health insurance, if eligible. The Health Educator will also help with testing, outreach, and care coordination.
Key Responsibilities
Outreach and Engagement
- Conduct street-based outreach.
- Assist with off-site testing program
- Establish and maintain trusting relationships with patients to support their engagement in healthcare and supportive services.
- Provide health education and information on available clinic and community services.
Care Coordination and Patient Navigation
- Provide follow-up support to ensure patients remain engaged in care.
- Assist with the Medication Assistance Program for program patients
- Provide support and guidance to all referrals seeking linkage to PrEP, PEP, STI testing and other medical providers.
- Introduce prevention services ensuring access, follow up, and adherence to PrEP, etc., including supportive services that can enhance harm reduction, behavior change, and maintenance
- Assist clients as they move through the service delivery system, including follow-up with patient that have fallen out of service
- Navigate patients through the necessary steps to acquire insurance, as needed, including application assistance
- Advocate and act as a liaison between the patient and all relevant professionals through the PrEP program that includes prevention, testing, and appropriate treatment that is patient centric
- Educate clients and others about the HIV disease process and prevention
- Assist with the elimination of barriers, including but not limited to financial and transportation barriers
Administrative and Compliance
- Participate in staff meetings, trainings, and quality improvement activities.
- Uphold confidentiality and HIPAA standards at all times.
- Collect and report data on program activities
- Stay informed about current HIV/AIDS trends, prevention efforts and strategies
- Ability to interact successfully with diverse populations
- Maintain highest level of client confidentiality; follow HIPAA regulations
- Attend team meetings and training programs
Other Duties
- Perform all other relevant duties as assigned.
Qualifications
- 5 years work experience working in HIV/STI prevent or other community health programs preferred.
- Certified in HIV/STD testing or willing to become certified within 30 days of hire
- Knowledge of local community/ socially connected to the communities of Imperial Beach and South San Diego.
- Ability to communicate effectively in both English and Spanish preferred.
- Cultural competency.
- Ability to maintain accurate and complete written logs of all activities.
- Access to reliable transportation.
- Ability to travel to scheduled events as required
- Ability to work independently and work collaboratively in a team
- Ability to work flexible hours.
- Ability to work independently.
- Comfortable speaking to groups and meeting new people

Working Conditions
- Work is performed in both clinical and non-clinical settings.
- Requires the ability to walk and stand for extended periods and navigate outdoor environments.
