Behavioral Health Case Manager

Grade/Level: Amount of Travel Required: up to 50% 

Work Schedule: Positions Supervised: None

8 a.m. to 5:30 p.m. Monday- Thursday. 

8 a.m. to 12:00 p.m. on Friday. 

 

8:00 a.m. to 4:30 p.m. (UAM Clinic and other school-based clinics) 

However, adjustments in schedule or additional hours may be required to complete duties. Schedule may differ based on clinic location and patient need.  

 

POSITION SUMMARY:

The Behavioral Health Case Manager reports to the Behavioral Health Director and is responsible for providing administrative support to the Behavioral Health Unit and Case Management to Integrated and/or Behavioral Health Patients and overall support to achieve Behavioral Health program goals. The Behavioral Health Case Manager instructs individuals and families about resources, referrals and access to services utilizing a team-based model. The Behavioral Health Case Manager will also complete clerical and administrative tasks to best serve patients. The Behavioral Health Case Manager is involved in continuous Quality Improvement practices and measures as set by Mainline Health Systems, Inc. and in which the practice focus is based on a Patient Centered Medical Home model of care. 

RESPONSIBILITIES:  

Include but not limited to:

  • Clerical and administrative tasks related to patient care such as scheduling appointments, reminder calls, outreach letters, coordinating integrated, behavioral health and medical appointments, updating patient contact information etc. and prior authorizations if indicated. 
  • Manage referrals and calls related to integrated care and behavioral health in a timely-manner.
  • Provide education to patients about MHS services as needed.
  • Assist with care coordination (connecting patients with needed resources, referrals to other agencies as needed, obtaining ROI’s as needed etc). 
  • Complete screeners to determine patient and family needs and develop plans to meet identified needs. Enter screeners in visit note/smart forms.
  • Assist providers with patient care by making follow-up calls to assist with patient adherence to treatment plan goals. 
  • Assist with coordination of Integrated Care services by monitoring and working with various disciplines on the unified treatment plans.

                                                                                                                                                                             

  • Assist Behavioral Health Consultant’s with patient care by monitoring, updating and tracking identified treatment goals.
  • Participate in Quality Improvement, Quality Assurance activities as appropriate for departmental tracking.                                                                                                                                                              
  • Serve as part of the care team; following and implementing workflows that support and sustain MHSI Patient Centered Medical Home model of care.
  • Participates in all Risk Management activities provide as assigned.

Qualifications

  • Strong reading, writing and communication skills 
  • Excellent interpersonal skills 
  • Excellent organizational and planning skills
  • Excellent professional customer service skills required
  • Attention to detail and accuracy
  • Strong computer skills
  • Working knowledge of relevant software, such as MS Office products and eClinical
  • Experience in linking patients with various community resources and patient enabling services preferred
  • An Associate is required. Bachelor’s Degree in psychology or social work preferred 

 

REPORTING RELATIONSHIPS:

Reports directly to the Behavioral Health Director 

 

EVALUATION:

Annual evaluation performed by Behavioral Health Director. 

 

STATUS:

This is a FLSA Non-Exempt position.

This is an OSHA low risk position.

This is an ADA negotiable position.
