JOB SUMMARY:

The Inmate Transition Program LES is specialized clerical work of a complex nature that is responsible for assisting program coordinators with evaluating incarcerated individuals’ needs prior to release. Interacts with inmates, and community and county service providers to coordinate release programming and avoid gaps in care, assistance, and treatment. Enters and maintains detailed records of all individuals served and services offered.

This is a Grant funded position for 1 year.

ESSENTIAL JOB FUNCTIONS:

Provide in person and telephone reception for visitors and callers; provide general information as appropriate, screen and direct calls and visitors to appropriate party; and transfers calls to 911 as necessary; answer questions regarding bonds, inmate’s status, case numbers, jail policies, arresting officer and other arrest information; help resolve issues with the public as necessary.

Monitor documents, files, and computerized data to ensure the integrity of the Mesa County Sheriff’s Office records and statistical information for which the public may have access.

Attend initial court arraignment sessions to determine individuals’ needs prior to release, and assist with services such as obtaining proper identification, treatment services, transitional housing, medical care, food assistance, and employment. 

Assist individuals with obtaining or verifying Medicaid enrollment.

Assist detention facility staff with coordinating inmates’ online GED education programming.

Work with the County and Community assistance organizations to identify availability of services, scope of assistance and points of contact.  Assist inmates with referrals and proper paperwork for the assistance.

Must collaborate effectively as part of an interdisciplinary team, including jail personnel.

Attend and participate in meetings pertinent to the position.

Participate in required trainings to keep updated on Detention operations, inmate programs and suicide prevention.

Create and complete monthly data tracking of all inmates assisted by the transition program to track success and recidivism rates.

In conjunction with Transition team coordinators, create a monthly report for data collected from internal and external assessments, needs identified, and include services or resources offered.

Must complete all required paperwork accurately, legibly, and within established timelines.

Performs other related duties as assigned. 

MINIMUM QUALIFICATIONS REQUIRED:

Education and Experience:

Bachelor’s degree or higher preferred, but not required. 

At minimum, a high school diploma or equivalent, plus any combination of education, training and experience which provides the knowledge, skills and abilities required for the job.

Licenses and Certifications:

Must have or be eligible to obtain a valid Colorado Driver’s License upon hire.

CBI/CCIC/NCIC Certification (must be obtained after hire)

PHYSICAL REQUIREMENTS AND WORKING ENVIRONMENT:

This position requires standing, stooping, sitting, bending, twisting, and lifting up to 10 pounds.

May be subject to dangerous situations and/or individuals.  May be stressful due to the nature of personal contacts with inmates.

May be exposed to communicable diseases, infections, blood and bodily fluids.

Must be available for shift and weekend work.
