Our goal is to keep mixed-Medicaid eligible families visiting the doctor together and to provide continuity of care as individuals fall in and out of eligibility.
The Community Health Lead is responsible for overseeing and coordinating the activities of the Community Health Navigators. This position involves leadership, program development, and collaboration with community stakeholders to address health disparities and improve the overall health and well-being of our patients. Additionally, the manager, through the team of navigators will have direct ownership over patient retention and satisfaction.
Supervision & Leadership
- Provide leadership, guidance, and support to a team of Community Health Navigators
- Conduct regular team meetings and trainings to enhance skills and knowledge
- Facilitate cross-cultural communication training for the team
- Foster a positive and collaborative team culture
Program Development
- Develop and implement the ¡Vamos! Patient Navigator programs and initiatives tailored to patient needs
- Establish and maintain partnerships with local organizations, healthcare providers, and community leaders in order to address systemic health disparities for our patients
- Evaluate program effectiveness and make recommendations for improvement
Quality Assurance
- Monitor and assess the quality of Community Health Navigator Interventions and community outreach activities
- Implement quality improvement initiatives based on data and feedback
- Ensure adherence to relevant policies, procedures, and standards
Data Management & Reporting
- Oversee data collection and reporting processes to track patient health trends and outcomes
- Prepare and present regular reports for stakeholders
- Ensure the confidentiality and accuracy of collected data
Resource Management
- Manage resources efficiently, including budget oversight and allocation of staff resources
- Coordinate the procurement of materials, supplies, and resources needed for ¡Vamos! Patient Navigator programs
Education:
Bachelor's degree in public health, community health, social work, or a related field
Experience:
5+ Years in community health, public health, etc.
Skills:
- Proficiency in English and Spanish (written and spoken)
- Previous experience in a supervisory or management role, preferably in community health or a related field
- Several years of experience in community health work, program development, and management
- Strong understanding of community health, health promotion, and cultural competency
- Excellent organizational, communication, and leadership skills
- Community Health Worker Certification
- Frequent use of a computer
- Varying schedule to include evenings, holidays, and extended hours as business dictates
- While performing the duties of this job, the employee is occasionally exposed to heat or cold temperatures.
