Providence Swedish Rehabilitation Hospital is a brand new, state of the art, 40 bed rehabilitation hospital.  We provide top quality rehabilitation to help our patients return to their homes and their communities.

How you'll contribute

The Case Manager plays a vital role in coordinating patient access to care by responding to inquiry calls, facilitating assessments, and ensuring appropriate referrals within the community or facility. This role involves conducting or overseeing comprehensive assessments to identify medical, behavioral, and psychosocial needs. The Case Manager collaborates closely with the clinical team and on-call physicians to ensure timely and effective treatment planning for patients.  Additionally, the Case Manager provides therapeutic support, educates patients and families to promote understanding and engagement in care, and manages administrative tasks such as admissions and pre-certification with external payers.

What we're looking for

  • Bachelor’s degree in social work, counseling, or similar is required.
  • Prior experience in a psychiatric healthcare facility working with adult and geriatric patients preferred.
  • Must have strong clinical assessment skills.
