Position: Case Manager 2

Location: New Haven

Salary: $22.00/hour

Schedule: Full time, 40 hours, 1st shift, Monday-Friday


Program Summary:


The Housing Empowering Recovery Opportunities (HERO) program is a permanent supportive housing program that works with homeless individuals with substance use diagnoses by assisting them in moving into permanent housing and providing intensive case management services. Services include providing in-home individualized case management, coordination of referrals and appointments, development of tenancy-based skills, landlord mediation, apartment searching, and life skill building activities. The program's goal is to help people maintain their subsidized housing and improve their day-to-day functioning.


Position Summary:


The Case Manager 2 will deliver comprehensive, home-based services to clients in order to assist clients in obtaining and maintaining permanent supportive housing, case management services can include job development and job search assistance, educational services, substance abuse education, life skills training, and client advocacy and crisis intervention. The Case Manager reports to the Program Manager.

Working within our Housing and Homeless Services, the Case Manager 2 works with the HERO program staff to provide services to stabilize and reunite families experiencing complex issues such as substance use, behavioral health matters, and homelessness. 


Requirements:


  • Associate’s Degree
  • 2+ years relevant experience in direct service and case management field
  • Valid Connecticut Driver’s License
  • Regular and predictable attendance is required

Orientation:


If selected for employment with The Connection, newly hired employees will be required to attend a two (2) day mandatory New Hire Orientation at our Roscommon office located in Middletown.
