About the Department

Catawba County Public Health is seeking a compassionate and detail-oriented nurse to join our Care Management for At-Risk Children (CMARC) Program. This position focuses on identifying and providing care management to children from birth to age five who have special health care needs or are exposed to toxic stress in early childhood. The Public Health Nurse in this role promotes the health and well-being of young children by helping families access services, navigate community resources, and address social determinants that impact their children’s development and overall health.

Position Duties

  • Identify potential clients and determine eligibility for CMARC services through assessment, screening, and professional judgment
  • Manage a caseload of CMARC patients residing in Catawba County, providing comprehensive care coordination regardless of where they receive well-child care (in or out of Catawba County)
  • Perform comprehensive health and developmental assessments using evidence-based tools such as the CMARC Comprehensive Needs Assessment, Life Skills Progression, and SWYC
  • Provide education, resource linkage, and referrals to families, while documenting services and outcomes in Virtual Health and other required systems
  • Independently plan and schedule contacts and home or practice visits according to risk level, family needs, and program requirements
  • Collaborate with healthcare providers, social services, early intervention programs, and other community partners to ensure coordinated care and follow-up
  • Engage in outreach to increase community awareness of the CMARC program, eligibility criteria, and available supports
  • Participate in ongoing program evaluation and quality improvement through audits, annual reviews, and the development or revision of policies, procedures, and educational materials
  • Make decisions based on independent judgment, giving account for actions and decisions, and consult with the supervisor as needed, especially when a situation might have legal implications
  • Maintain knowledge of available local and regional agencies and resources that serve families and children, help develop new resources as unmet client needs emerge, and coordinate these services for the client
  • Maintain and enhance professional knowledge in areas related to early childhood health, development, and family care management

Minimum Qualifications

  • Bachelor's or Associate’s degree in Nursing from an accredited institution and 1 year of professional nursing experience
  • Must be licensed to practice as a Registered Nurse in North Carolina by the NC Board of Nursing

Other Qualifications

  • A valid Driver’s license with a safe driving record (Driving is an essential function of this position)
  • Individuals employed in a local health department without a degree that includes a Public Health Nursing rotation will be required to complete the "North Carolina Credentialed Public Health Nurse" course within one year of employment
  • Current certification in Basic Life Support (CPR/BLS) or ability to obtain within the specified timeframe after hire
  • Strong assessment and case management skills, with the ability to identify abnormal findings or concerns in children aged 0-5 and postpartum women
  • Excellent communication, organization, and time-management skills
  • Knowledge of community resources and ability to advocate effectively for families
  • Demonstrated capacity to work independently, exercise sound nursing judgment, and collaborate as part of a multidisciplinary team
  • Creativity and flexibility in problem-solving and supporting culturally diverse populations
  • This position is considered Medium Work, requiring the ability to lift 30 pounds frequently to lift small children and equipment during assessments.  


Preferences:

  • Current certification in Basic Life Support (CPR/BLS)
