🕒 Schedule: Exempt, min 30 - 40 hours per week | Monday - Friday, 8a - 4:30pm with flexible work options available

🎯 What You’ll Do in This Role:

The Primary Care Physician performs initial and follow up comprehensive health assessments for participants, documents thoroughly, and participates actively in Interdisciplinary Team (IDT) meetings to help develop, implement, and evaluate individualized care plans. Educates participants and families on managing chronic conditions, adjusts medications in collaboration with pharmacists and specialists, and oversees care transitions, including regular visits and communication with skilled nursing facilities. Works closely with the Medical Director and Advanced Practice Professionals to ensure coordinated, high-quality care.

Primary Responsibilities:

Clinical Assessment & Documentation:

  • Performs comprehensive history and physical assessments for new enrollees and conducts periodic evaluations for established participants.
  • Documents all assessments and care in the medical record per regulatory and organizational requirements.

Interdisciplinary Team Collaboration & Care Planning:

  •  Actively participates as a member of the Interdisciplinary Team (IDT), providing regular updates and communicating participant status changes.
  • Assists in the development, implementation, and evaluation of individualized care plans based on participant preferences and clinical needs, following LifeCircles policy.

Chronic Disease Management & Education:

  • Educates participants and families on chronic condition management and health maintenance.
  • Participate in family conferences to support shared decision-making and goal setting.

Medication Management:

  •  Adjusts and regulates medications in response to participant health changes.
  • Collaborate with pharmacists to ensure safe and effective medication use.

Specialty & Transitional Care Coordination:

  • Coordinates with medical specialists when indicated and oversees care provided by consultants.
  • Manages care during skilled nursing facility (SNF) admissions, including regular evaluations and communication with SNF staff.

Medical Team Collaboration:

  • Works closely with the Medical Director and Advanced Practice Professionals to provide comprehensive, coordinated care.

 What You’ll Need:

Education & Certification:

  • Graduate of an accredited School of Medicine or Osteopathy, required
  • Board certified in Internal Medicine or Family Practice, required
  • Unrestricted license to practice medicine in the state of Michigan, required
  • Advance certification in geriatrics and/or palliative medicine, preferred
  • A minimum of 1-year experience working with frail elderly, preferred
  •  Current CPR certification

Skills & Competencies:

  • Ability to lift 50 pounds without assistance
  • Strong clinical assessment and decision-making skills.
  • Knowledge of geriatric care and psychosocial determinants of health.
  • Effective communication and collaboration skills within an interdisciplinary team.
  • Familiarity with electronic health records (EHR) and care coordination workflows.
