- Conduct thorough assessments to identify clients’ risk factors for relapse, including personal history, triggers, and coping mechanisms.
- Provide one-on-one counseling sessions to develop personalized relapse prevention plans, emphasizing self-awareness, coping skills, and stress management.
- Lead educational and support groups focused on relapse prevention techniques, including recognizing triggers, developing coping strategies, and building a support network.
- Lead care conferences with required stakeholders and provide progress, insights on current barriers and updates on compliance.
- Offer immediate support and intervention in crisis situations, employing de-escalation techniques and ensuring the safety of clients.
- Work closely with other behavioral health team members, including therapists, medical staff, and social workers, to provide comprehensive care.
- Educate residents and their families about the recovery process, including the importance of aftercare and ongoing support networks.
- Maintain accurate and up-to-date client records, treatment plans, and progress notes in compliance with facility policies and regulations.
- Stay informed about the latest research and best practices in addiction treatment and relapse prevention.
- Knowledge of addiction theories, recovery processes and evidence-based practices in relapse prevention.
- Able to manage and provide redirection of potential behaviors of residents in a safe and effective manner.
- Ability to create care plans and effectively document changes in condition
- Possesses superior interpersonal, human relations, and behavior management skills.
- Proficient in crisis intervention and conflict resolution.
- Ability to quickly build rapport and trust with residents.

- High School Diploma or equivalent required.
- Associate or bachelor’s degree in field of Psychology, Counseling, Social Work or related field preferred.
- Certified Alcohol & Drug Counselor Level I (CADC-I) preferred.
- Minimum of 2 years of experience in addiction counseling or related field with a focus on relapse prevention preferred.
- Experience working with adults with developmental disabilities and/or moderate to severe mental illness preferred.
