Description
JOB PURPOSE: To facilitate and foster relationships and awareness of home health care and maintain convenient access to care, treatment and services. Establishes and maintains working relationships and open lines of communication between appropriate personnel within the home health program of the agency, physicians within the community, discharge planners and other healthcare providers within the community. To secure accurate and timely physician orders to facilitate documentation for compliance and reimbursement. To act as a resource person for community contacts regarding home care coverage and services.
QUALIFICATIONS:
- Registered nurse preferred.
- Current Maryland license as appropriate.
- Two to three years healthcare experience or related field, home health care experience preferred.
- Strong communication skills including written and verbal; computer literacy required.
RESPONSIBILITIES:
- Interacts with health care team members, clients and their families to answer questions regarding home health services.
- Informs and educates community regarding the scope and goals of home health services, including specifics of covered services, medical necessity and supportive/personal care available.
- Facilitates patient/client access to home care services from point of referral to admission,
- Communicating and coordinating care, treatment and services with multi-disciplinary team.
- Establishes rapport with patient/family reviewing eligibility, payer/financial obligation,
Options and baseline risk factors such as, learning deficits, fall risk, environmental concerns, language barriers and/or caregiver support.
- Manages physician relationships, effectively communicating physician rights and responsibilities to ensure timely verification of orders and continuity of patient care, treatment and services.
- Participates and/or facilitates community outreach including health fairs, speaking engagements, physician presentations/education.
- Remains current regarding community needs and the market trends impacting home health care services and develops strategic marketing plans designed to meet the needs, trends and agency goals.
- Contributes to effective customer relations and community outreach by assisting all customer constituencies to resolve expressed concerns and by demonstrating a positive and helpful attitude.
- Assumes the administrative functions of the agency under the direction of the Executive Director.
- Performs other duties as assigned.
Requirements
SPECIAL REQUIREMENTS
- Possess and maintain documentation of freedom of TB in a communicable form.
- Possess and maintain documentation of annual influenza vaccination.
- Mandatory Competencies as defined by the agency Education Plan
- Maintain current CPR certification.
- Maintain current driver’s license.
- Maintain current auto insurance of at least $100,000 total liability insurance per person and $200,000 total liability insurance per accident.

PHYSICAL DEMANDS:
The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
While performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling and stooping all day. The employee must frequently lift or move items weighing over 20 pounds. This position requires the ability to transfer, lift, move, and turn patients to and from bed, wheelchair, toilet, tub, shower, and occasionally the floor.
WORKING CONDITIONS:
- Work is performed primarily in the office but occasionally in the patient’s home or other healthcare settings. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
- Exposure Risk to Blood borne Pathogens: Category A - routine exposure to blood and body fluids. Occasional exposure to hazardous materials and infectious diseases.
- Travel may be required at times, sometimes in hazardous weather conditions.
EMERGENCY RESPONSIBILITY:
Expected to respond to emergency situations involving the safety of the patient in their homes. This includes the ability to assist with a possible evacuation of patient, or notify the proper agency/facility in case of any type of emergency situations such as calling an ambulance or the fire department.
