Patient Care
- Perform comprehensive head-to-toe assessments and reassessments per policy for adolescent, geriatric, and general patient populations.
- Assess and manage pain effectively using appropriate techniques; educate patients and families on pain management strategies.
- Revise and evaluate the plan of care daily based on patient response to treatment.
- Deliver age-appropriate care tailored to the patient’s condition and developmental stage.
- Administer medications accurately, considering patient age and clinical condition; adhere to the five rights of medication safety.
- Develop individualized teaching plans based on identified learning needs; evaluate effectiveness and include family education when appropriate.
- Assist physicians with procedures and perform technical and manual nursing tasks within licensure scope.
- Provide treatments and services consistent with licensure and clinical competencies.
- Treat patients and families with respect and dignity; address psychosocial, cultural, ethnic, and spiritual needs.
- Act as a liaison among administration, patients, physicians, and other healthcare professionals.
- Engage patients and families in care planning and maintain professional communication.
- Apply knowledge of growth and development to deliver age-specific care and interpret patient data accurately.
- Promote patient safety and minimize healthcare errors in all aspects of care.
- Collaborate with other departments to ensure an interdisciplinary approach to patient care.
- Identify and address psychosocial needs; coordinate with Social Services/Discharge Planners for in-hospital and post-discharge support.
- Supervise and coordinate patient care activities as needed.
- Maintain patient confidentiality at all times.
- Communicate clearly and appropriately with Nurse Manager, coworkers, and physicians.
- Foster positive working relationships within and across departments.
- Maintain clinical competency in stroke care; participate in ongoing education and complete required stroke-related competencies.
- Serve as a hospital-wide resource for stroke care, regardless of admitting diagnosis.
- Manage and coordinate stroke patient care during hospitalization and post-discharge.
- Ensure documentation meets current standards and organizational policies.
Safety Practices
- Promotes a culture of patient safety which results in the identification and reduction of unsafe practices.
- Utilizes Culture of Safety survey responses in improvement efforts.
- Identifies risks and hazards to patients through various risk assessments, with actions taken to mitigate patient harm.
- Improves medication safety practices, including increased reporting of adverse drug events and improved compliance with the medication reconciliation process
- Decreased, hospital-acquired infections and improved compliance with infection control practices (including hand hygiene)
- Decreased hospital acquired conditions such as complications associated with ventilator use and pressure ulcers, through improved compliance with patient safety bundles and/or other formal performance improvement programs.
- Manages and operates equipment safely and correctly.
- Demonstrates understanding of suicide prevention and patient restraints for sitting assignments of 1:1 care, frequent checks and continuous observation of patients.

Education
- Associate Degree in Nursing
Experience
- Minimum 2 years’ experience in Nursing units- Preferred
