Reporting to the Director of Hospice, the Home Care/Hospice on call nurse, provides on-call coverage for the Agency’s Home Care and Hospice programs during the hours that the agency is closed and completes hospice visits as needed.
Upon request of the Administrator On-Call will assist the other programs.
Essential Functions

- Responds to and/or triage calls during on-call hours in a timely manner and documents all telephone contacts, patient care observations and interventions.
- After triaging calls, document outcomes of call along with patient/caregivers responses and if any further instruction is needed in Electronic medical record system.
- Prioritizes calls and completes visit for patients as needed. Communicates with and supports after-hours staff in assessing and treating patient's needs.
- Documents all interim verbal orders obtained from the physician accurately into the electronic medical record system and communicate to all members of the team to ensure implementation.
- Provides skilled professional nursing care to patient/family/caregivers as prescribed by the physician and in compliance with agency policies when called upon after-hours.
- Completes clinical documentation of all after-hours calls and visits, including problems and interventions within the Electronic medical record system timely and in accordance with agency policies and procedures.
- Completes an on-call log on telephone calls as received. Submits on a daily basis.
- Communicates with the interdisciplinary team to ensure proper patient follow-up on previous night's after-hours calls.
- Provides accurate information to customer making telephone inquiries regarding Home Care. Hospice, and other agency services.
- Notifies after-hours administrator on-call when necessary and/or when guidance needed.
- Communicates clearly with professional staff regarding expected "call-backs" and relays information to physicians and others.
- Maintains a positive working relationship with referral sources and individuals seeking information.
- Safely operate a personal vehicle while traveling throughout Granite VNA service area to provide patient visits and/or attend meetings.
Other Job Functions
- Demonstrates flexibility and accepts other duties as assigned
- Demonstrates knowledge of Medicare Regulations and delivers care in accordance with Medicare Condition of Participation (COP) for hospice/home care.
- Arranges to have needed supplies and equipment in the patient's home. Appropriately uses supplies and equipment. Seeks out education for any unfamiliar procedures.
- Performs job duties in a safe manner in order to prevent injury to yourself and others
Requirements
- Graduate of an accredited school of nursing, accompanied by current New Hampshire nursing license, or compact state license through the Nurse Licensure Compact (NLC).
- Three to four years of recent general medical/surgical or skilled/acute care nursing experience.
- Valid driver's license, reliable transportation and an acceptable driving record.
Expected Hours of Work
The team member will be scheduled to work based on program and Agency needs. Work assignments may include holiday, weekend rotations. This is a remote position for triage responsibilities during on call hours, and located in the community providing direct patient care as needed.
