Schedule: Per Diem/Flex | Varies-must be able to work 3 (three) days per month, with 2 (two) days being a weekend shift & 2 Holiday rotations required.

 Shift Differentials, Holiday, and Weekend Differentials: Nights (11:00 pm-6:59 am)-$2.00, Weekend (Fri 11:00 pm-Sun 10:59 pm)-$0.50

💰Pay Range: $34.61 - $40.85/hr. (this rate is inclusive of the $3.40/hr. Michigan Skilled Nursing Facility Direct Care Worker Pass-Through approved rate; current approval through September 30, 2026)

🎯 What You’ll Do in This Role:

As a Graduate LPN, you’ll work under the supervision of a Registered Nurse or Licensed Practical Nurse to provide exceptional care and support to residents in our Skilled Nursing Facility. You’ll play a vital role in promoting residents’ independence, dignity, and overall well-being while gaining valuable experience as you work toward full LPN licensure.

Primary Responsibilities:

  • Provide individualized nursing care that supports residents’ physical, mental, and emotional health.
  • Assist residents in maintaining dignity, comfort, and independence in a Christ-centered environment.
  • Communicate effectively with residents, families, and healthcare professionals.
  • Administer medications and treatments as directed, ensuring accuracy and adherence to policy.
  • Observe and document changes in residents’ conditions and promptly report to supervising nurses or physicians.
  • Support a safe, clean, and homelike environment for all residents.
  • Participate in resident assessments, care planning, and shift reports.
  • Follow safety and infection control procedures, including proper use of PPE and hygiene practices.

 What You’ll Bring:

  • Successful completion of an accredited LPN program.
  • Actively pursuing Michigan LPN licensure; must pass the licensing exam within 45 days  of hire.
  • Previous experience as a CNA in a skilled nursing setting preferred.
  • Current CPR certification.
  • Experience with Seniors: Prior experience working with elderly individuals or in geriatric care is highly valued.
  • Physical Requirements: Must be able to lift and move up to 35 pounds independently.
