The Prince George’s County Health Department is currently seeking qualified applicants to fill a Community Health Nurse(Community Health Nurse I/IIgrade A18-A21 position, in the Health and Wellness Division.


About the Position

This is a full performance level Professional Registered Nurse who provides nursing services and conducts various types of assessments for chronically ill and elderly residents. The Community Health Nurse in this position conducts home visits to assess the health care needs of the individual and identify services for individuals who are at risk of Nursing Home Placement. The population served are infants, youth, adults and seniors.

The PGCHD works to protect and support the public’s health through numerous services that range from restaurant inspections and disease tracking to care coordination and health promotion. The department is composed of four divisions: Behavioral Health Services, Environmental Health and Disease Control, Family Health Services, and Health and Wellness, supported by the Office of Administration, Office of Human Resources and the Office of the Health Officer. The mandate of the department has been, and will remain, broad. This plan emphasizes improving core functions and operations, including protecting the public from health threats, implementing strategies to support healthy living, and maintaining a focus on vulnerable populations, while developing efficient internal processes to support these activities.

Position Duties

  • Provides direct nursing services to individuals utilizing the nursing process by conducting a comprehensive evaluation to determine medical needs and establishing a Person-Centered Plan of Care. 
  • Teaches and demonstrates care procedures and health promotion concepts to clients and their families. 
  • Document assessments, treatment, intervention and progress in agency medical records in accordance with Maryland Department of Health (MDH), Health Department (HD) and Federal guidelines. 
  • Confers with health professionals, Supports Planners, family members and other designated individuals on the care needs of clients. 
  • Complete weekly/monthly reports.

Minimum Qualifications

Minimum Qualifications

  • Graduation from an accredited school, college, university or an equivalent program with a bachelor's degree in nursing or a related health field or an equivalent combination of education and experience.
  • Possession of a valid license to practice as a Registered Nurse in the State of Maryland issued by the Maryland State Board of Nursing at the time of appointment.
  • Possession of a valid license to operate a motor vehicle when required for the performance of job-related duties at time of appointment.
  • Possession of a valid cardiopulmonary resuscitation (CPR) certification.
Preferred Qualifications
  • 2 years of home visiting experience. 
  • Proven experience and desire to work in a team oriented, fast-paced and professional environment. 
  • Ability to communicate effectively, orally and in writing.

  • Be willing and able to serve as an essential employee. Essential employees are expected to report during standard or non-standard hours as operations necessitate, or during emergencies. Essential employees are expected to report or remain at work when other County employees are granted Administrative Leave.
