• Updated resume (if not already submitted)
• State Required License/Certification applicable to the job (CNA, LPN, etc.)
• CPR/BLS/ACLS Card & Infection Control Certificate (For Nursing Only)
• Social Security Card (For W2 Tax Verification Purposes Only)
• I-9 Documentation (CLICK HERE TO VIEW APPROPRIATE I-9 EMPLOYMENT DOCUMENTATION)

(2) Copy of Immunizations/Proof of MMR Immunity, (3) TB test (Chest X-ray, PPD within the past year, or QuantiFERON Blood test), (4) Proof of COVID vaccine status (if you received), (5) Proof of Flu Vaccine (We can give to you free of charge if you are opting to receive it).
