Health Support Assistant
QUALIFICATIONS: Licensed Practical Nurse
      Pediatric experience preferred
      Such alternatives to the above qualification as the 
      BOE may find appropriate.
EMPLOYMENT TERMS: Contract to be determined by
the School Board
REPORTS TO: District School Nurse and Director
of Special Education
JOB GOAL: Work with the District School Nurse to
support the daily health needs of the students
and to facilitate a healthy, safe educational
environment for all the students
EVALUATION: Performance of this job will be evaluated
in accordance with the Board’s policy on Evaluation
of Classified Personnel
PERFORMANCE 
RESPONSIBILITIES:
Assists the District School Nurse in performing the following tasks:
• Maintains records and performs clerical work
• Administers first aid in accordance with established
      first aid procedures
• Conducts vision screenings 
• Refer students in need of medical and dental care
• Maintains up-to-date health records for students
• Assumes responsibility for injured or ill students
• Contacts students parent/guardian as necessary
• Assists in preparation of reports
• Follow-up exclusion and re-admission of students in
      connection with infectious disease
• Assists school personnel in establishing sanitary conditions
• Dispense medication as ordered
• Chart vision screening results on all permanent records
      of students
• Record daily diabetic screening results
• Follow physician orders caring for diabetic students daily
needs while optimizing the educational experience
      of each student
• Assists sending referral letters to parents following
      failed vision screenings
• Ensure safety of students
• Make contact with the public utilizing tact and diplomacy
• Follows strict adherence to student confidentiality following
      FERPA policy and HIPAA
• Interact in a positive manner with staff, students, and parents
• Attend meetings and in-services as required
• Keep daily clinic log of students requiring health assistance 
• Assist with other health screenings as needed 
• Coordinates school health resources and school health services
      to identify and prevent adverse pupil health outcomes
• Conducts home visits if applicable 
• In-service staff about student medical conditions and
procedures and determine competency to perform
      said procedures as needed
• Comply with all Plain Local Board of Education policies
and procedures
