On-Call Schedule:
- Available for visits from 5pm to 8:30am, 7 days on, 7 days off.
Day-to-Day Life at Custom:
- Conduct initial and ongoing comprehensive assessments of patients and families to determine hospice needs.
- Provide professional nursing care using the full nursing process.
- Collaborate with interdisciplinary teams to develop and revise care plans based on patient needs.
- Administer medications and treatments as prescribed by physicians.
- Educate patients and families on self-care techniques and health management.
- Coordinate patient care and maintain accurate clinical notes.
- Attend interdisciplinary group meetings and participate in on-call duties as needed.
- Ensure a safe environment for patients and provide emotional support to patients and families.
- Actively participate in quality assessment and performance improvement activities.
What We Are Looking For:
- Graduate of an accredited school of nursing with 1-2 years of recent acute care experience.
- Current licensure in the state and a valid driver's license.
- Bachelor's degree preferred, with 1 year of hospice care experience.
- Excellent communication, problem-solving, and nursing skills.
- Ability to lift and transfer patients, with physical stamina for prolonged standing and walking.
