On-Call Schedule:

• Available for visits from 5pm to 8:30am, 7 days on, 7 days off.

Day-to-Day Life at Custom:

• Conduct initial and ongoing comprehensive assessments of patients and families to determine hospice needs.
• Provide professional nursing care using the full nursing process.
• Collaborate with interdisciplinary teams to develop and revise care plans based on patient needs.
• Administer medications and treatments as prescribed by physicians.
• Educate patients and families on self-care techniques and health management.
• Coordinate patient care and maintain accurate clinical notes.
• Attend interdisciplinary group meetings and participate in on-call duties as needed.
• Ensure a safe environment for patients and provide emotional support to patients and families.
• Actively participate in quality assessment and performance improvement activities.

What We Are Looking For:

• Graduate of an accredited school of nursing with 1-2 years of recent acute care experience.
• Current licensure in the state and a valid driver's license.
• Bachelor's degree preferred, with 1 year of hospice care experience.
• Excellent communication, problem-solving, and nursing skills.
• Ability to lift and transfer patients, with physical stamina for prolonged standing and walking.
