Abstraction of records real-time nd the abstraction of information from the medical records of discharged patients. Enters information abstracted from medical records onto screening tools and/or into data base programs as specified. RN licensed to practice in Arkansas. Three to five (3-5) years acute care care experience. Excellent interpersonal and communication skills (oral and written) required. Data management skills required. Computer skills including word processing and data base programs required. Core measures/quality management skills. 

• Daily abstraction of ED visits & direct admits for signs & symptoms of Sepsis
• Daily abstraction of mothers & newborns for PC-01, 02, 05
• Weekly entry into Premier website for IP-PC, IP-SEP, IP-SDOH and OP-ED throughput, OP-STK. Also, web based OP-29.
• Monthly entry into NHSN to add new employees/delete termed employees and send COVID numbers
• Annual entry into NHSN for ‘Influenza Among Healthcare Personnel’ (HCP)
• IQI Medicaid Inpatient Quality Incentive program from Jul 1 thru Dec 31 annually. Abstraction & entry of Behavioral Health Screening, Elective Delivery, Primary C-Sections, Opioid Use, Exclusive Breastfeeding, Tobacco Treatment and Syphilis Screening.
• Submit chart-abstracted data to The Joint Commission’s Direct Data Submission

Platform (DDSP) quarterly.

• Hospital Quality Reporting Secure Portal (HARP)…monitor site to be sure that data for PC-01, eCQMs, structural measures, population & sampling, DACA, THA/TKA PRO-PM measures are transmitted within this portal.
• Assist with focused reviews as needed
• Reporting hospital-wide deaths to Mid-South Transplant Foundation monthly
• Mortality Report in quarterly Medical Care committee
• Freedom Sleep reporting in quarterly Medical Care committee
• Reporting perinatal measures & Social Drivers Of Health (SDOH) in quarterly Surgery-Perinatal committee
• Attend Utilization Management committee…report SEPSIS, SDOH
• Initiate quarterly Sepsis Committee
