POSITION DESCRIPTION:
Provide social work intervention to SIHF Healthcare patients consistent with SIHF Healthcare policies and procedures, professional practice standards, and state and federal regulations.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Provide direct service intervention to best meet the needs of patients served by screening/assessing each patient that is identified as at risk to ensure appropriate ongoing health care and management of social/psychological needs, including advocacy to protect the rights and best interest of each patients served.
2. Provide ongoing follow-up to patients and their subsequent dependents. These patients will be identified at risk to ensure appropriate ongoing health care and management of social/psychological needs.
3. Support the mission of the organization’s served by example.
4. Practice directives regarding confidentiality consistent with HIPAA, JCAHO, and other state/federal regulations.
5. Participate in performance improvement activities and follow the established improvement process as deemed appropriate by management and to management satisfaction.
6. Demonstrate own professional development and leadership skills to ensure quality patient care in accordance with accepted professional standards as defined in policies.
ADDITIONAL POSITION DESCRIPTION:
1. Provide services to other categories of patients with social/psychological needs when necessary, to the satisfaction of the manager/ administration.
2. Maintain confidentiality of all information and records maintained in the Foundation consistent with HIPAA guidelines measured by zero breaches of confidentiality.
3. Use patient demographic information to prepare reports as required for the records committee function.
4. Demonstrate and promote positive relations with internal and external customers, staff, and co-workers in a friendly, caring, and timely manner in accordance with the Mission and Values of SIHF Healthcare.
5. Perform other related duties as required.
6. Receive guidance with respect to general objectives in the majority of tasks and projects assigned, determine methods, work sequence, scheduling, and how to achieve objectives of assignments; operate within specific policy guidelines.

EDUCATION AND EXPERIENCE:
Master's degree in social work from an accredited school of social work. Must be a licensed social worker or a licensed clinical social worker in Illinois. Experience in a health care setting is preferred. Maintain or advance in appropriate licensure. Must be able to provide competent services in an age appropriate manner to neonatal, pediatric, young adult, adult, and geriatric patients. Age-specific competency is measured by a variety of methods, which may include, but not limited to, self-learning packets, review of age-specific guidelines, direct observation, video, simulation, unit based class, policies and procedures, or chart review.

MEDICAL HOME STATEMENT:

PHYSICAL DEMANDS:
Light physical activity performing non-strenuous daily activities of an administrative nature. Must lift and carry records and materials weighing up to twenty (20) pounds; be able to work at a computer terminal and type on a keyboard for two hours a day; must be able to walk, stand, stoop, bend, and sit throughout the day to complete job duties; must have normal hearing and vision, corrected if necessary; must be able to type accurately; must be able to operate a telephone, copier, fax machine, and dictation equipment.
WORK SCHEDULE:
Normal work hours for this position are Monday through Friday dayshift; Saturdays two to three times per year (usually from 9:00 am until 1:00 pm). Late evening (7:30 p.m. at the latest) one to two times per week.
