About the Department
The purpose of this classification is to coordinate various programs/services in support of senior citizens, including services such as delivery of meals to homebound clients, housekeeping services, or coping with Alzheimer’s disease and other disorders.
Position Duties
The following duties are normal for this position. The omission of specific statements of the duties does not exclude them from the classification if the work is similar, related, or a logical assignment for this classification. Other duties may be required and assigned.
Coordinates activities of an assigned program/service designed to assist clients of senior center with various aspects of life.
Coordinates homebound delivered meals program for seniors; locates referrals for potential clients to receive homebound meal services; obtains and records directions to client homes; explains program and local guidelines prior to enrollment; completes referral forms, registration forms, and nutritional screening instrument; procures prescriptions for diabetic clients and places in client files; provides backup coverage for food preparation, meal delivery, client transportation, or other tasks.
Coordinates homemaking services for seniors; maintains waiting list of future clients; maintains reports/logs of activities.
Coordinates exercise, lunch, and snacks; works to provide clients with a relaxed atmosphere and a sense of security in surroundings and with volunteers; encourages clients to participate in sharing personal/family experiences; stimulates memory of personal life, birthdays, and happy times; coordinates short expeditions such as town tours, picnics, lake visits, or trips to ice cream store; carries clients to restrooms as needed; reviews medical publications relating to potential advances in the diagnosis and treatment of Alzheimer’s disease and other dementia-related disorders.
Maintains current files of each client and related forms/records.
Schedules and conducts regular reassessments of clients to ensure continued eligibility for program participation.
Prepares or completes various forms, reports, correspondence, client reports, waiting lists, referral forms, registration forms, nutritional screening instrument forms, calendars, mileage reports, reimbursement reports, or other documents.
Receives various forms, reports, correspondence, referrals, maps, medical resources, product instructions, policies, procedures, manuals, directories, reference materials, or other documentation; reviews, completes, processes, forwards or retains as appropriate.
Operates a computer to enter, retrieve, review or modify data; verifies accuracy of entered data and makes corrections; utilizes word processing or other computer programs.
Communicates with supervisor, employees, students, volunteers, other departments, clients, passengers, the public, community agencies, outside agencies, and other individuals as needed to coordinate work activities, review status of work, exchange information, or resolve problems.
Maintains a comprehensive, current knowledge of applicable laws/regulations; maintains an awareness of new trends and advances in the profession; reads professional literature; maintains professional affiliations; attends workshops and training sessions as appropriate.
ADDITIONAL FUNCTIONS
Operates a passenger van or small bus to transport clients or other individuals as needed.
This classification is designated as a safety sensitive position which is subject to random drug and alcohol testing and any other drug testing methods as stated in policies adopted by Forsyth County concerning drug and alcohol.
Performs general/clerical tasks, which may include answering telephone calls, making copies, sending/receiving faxes, filing documentation, picking up or delivering mail, or conducting visitor tours of senior center.
Assists with senior center activities and special projects as needed; serves as escort on trips as needed.
Provides assistance to other employees or departments as needed.
Performs other related duties as required.
Minimum Qualifications
High school diploma or GED; supplemented by one (1) year previous experience and/or training involving senior program administration, working with senior citizens, office administration, and experience in specific area of assignment; or any equivalent combination of education, training, and experience which provides the requisite knowledge, skills, and abilities for this job. Must possess or obtain valid CPR and First Aid certification within three (3) months of employment. Must possess and maintain a valid Georgia driver’s license. Assignment to the Senior Services grant programs of Meals on Wheels, Congregate, and Respite requires passing a federal fingerprint background check.
