Harbor’s Edge is a premier 5-star, resort-style Continuing Care Retirement Community (CCRC) located alongside the beautiful harbor in Norfolk, Virginia. Our community consists of 300 Independent Living Residential Units, a 33-bed (Medicare/private) Skilled Nursing Healthcare Unit, a 40-bed Assisted Living Unit, and 34-bed Memory Support Unit.

We are seeking compassionate, highly qualified full-time and part-time LPNs to join our teams to deliver excellent care for our residents. The ideal candidates will have senior living experience (e.g., long-term care, assisted living, memory support) with a passion for working with an elderly population. Responsibilities: 

  •  Assist with assigning work schedules and duties; ensure appropriate staffing levels, including coverage for callouts. Orient and train new employees, if directed by the AL/MS Manager.
  •  Make rounds. Assist in patient care as needed. Advise AL/MS Manager, or designee, of status of seriously or critically ill patients and unusual situations. 
  •  Audit charting. Review all medical documentation submitted by staff (i.e., MARS/TARS/POC) for consistency with sound medical practices and regulatory compliance to include physicians’ orders, and ISPs.
  •  Assess residents for admission by reviewing physician documentation.  Assist with the admission and discharge process. 
  •  Assist with transportation, consultation appointments, and family notification. Follow up on consultations/reports.  
  •  Complete labs, skin assessments, Narcotic sheets, and nurse’s notes.  
  •  Complete appropriate forms and reports: e-calls, updates to annual regulatory forms assigned. 
  •  Perform other duties as assigned. Attending in-service classes and staff meetings. Participate in committees. 
  •  Respond to emergencies/codes. Prepare the paperwork and transportation. 
  •  May be required to work in other areas of health services, when needed 

Requirements

  • High school diploma or equivalent (GED) required. 
  • Minimum of two (2) years of relevant experience working with elderly individuals, particularly those with dementia. 
  • Required: Possess a current unencumbered license as a Licensed Practical Nurse from the State Board of Nursing of the Commonwealth of Virginia.  
  • Current certification in Adult CPR and AED required. 
  • At least 2 years of experience in the CCRC, Long term, or assisted living level of care industry or related field. 
  • Knowledge of nursing and medical practices and procedures, laws, regulations, and guidelines pertaining to long-term care. Knowledge of medical terminology, medical records management, medical supplies, and equipment. Experience with clinical and diagnostic procedures. Assessment and documentation skills.  
  • Moderate computer skills. Experience with use of electronic medical records.
  • Strong reading, writing, and communication skills; ability to follow both written and verbal instructions effectively. 
  • Demonstrated ability to interact with the elder population, staff, families, and the management team in a respectful, compassionate, and patient manner, while showing flexibility, adaptability, and strong interpersonal skills. 
  • Consistently upholds residents’ rights, showing integrity and discretion when handling personal and health-related information.
