Nurse Practitioner (APN) – Family Practice
Seeking a dedicated and compassionate Nurse Practitioner (APN) to join our family practice team of five physicians. This role offers an excellent opportunity to provide comprehensive primary care services in a supportive and patient-centered environment. The ideal candidate will be committed to delivering high-quality healthcare and building lasting relationships with patients of all ages.
Key Responsibilities:
- Take medical histories and perform physical examinations
- Diagnose and manage acute and chronic illnesses in a family practice setting
- Develop and implement individualized treatment plans in collaboration with patients and healthcare team members
- Prescribe medications and treatments in accordance with state and federal regulations
- Provide health education and counseling to promote wellness and disease prevention
- Order and interpret diagnostic tests and laboratory results
- Document patient encounters accurately and maintain comprehensive medical records
- Collaborate with physicians, specialists, and other healthcare professionals to ensure coordinated patient care
- Stay current with medical advancements and maintain licensure and certifications
- Perform basic procedures (Cryo, Skin Bx, I&D, etc.) If qualified
Skills and Qualifications:
- Master’s degree in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited program
- Current Nurse Practitioner license and certification in family practice
- Prior experience in an out patient and primary care or family practice setting is preferred
- Strong clinical assessment and diagnostic skills
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
- Knowledge of electronic health records (EHR) systems. (eCW experience a plus.)

Requirements
Clinical Experience
- 1–3+ years of experience as an NP in a primary care or family medicine setting preferred
- Experience with chronic disease management, preventative care and acute conditions at all ages
Skills & Competencies
- Proficiency in Electronic Medical Records (EMR) systems (eCW experience a plus)
- Strong knowledge of evidence-based clinical guidelines and value base care
- Ability to work independently and collaboratively within a care team.
- Strong communication and patient education skills.
- Culturally competent care and sensitivity to diverse populations.
Role Responsibilities
