The Certified Diabetic Educator is responsible for educating and supporting patients with diabetes through personalized care plans, nutrition and medication counseling, and training on diabetes management tools. Collaborates with healthcare providers, interprets clinical data, and ensures accurate documentation and compliance with clinic standards.
This is a part-time (0.6) position working weekdays between the hours of 7:30 am - 4:30 pm out of our Edina clinic.
Endocrinology Clinic of Minneapolis is an operating division of Infinite Health Collaborative (i-Health), a group of local medical practices, owned and led by its physicians, on a mission to give patients more control over their healthcare.
Essential Functions:
- Conduct face-to-face assessments and provide individualized diabetes education using AADE7 principles.
- Develop and implement DSMT-based care plans, including behavioral goal setting and progress tracking.
- Teach diabetes self-management to individuals and groups.
- Educate patients on carbohydrate counting, label reading, dining out, alcohol use, and meal planning.
- Counsel on medication dosing and nutrition strategies to manage blood glucose, lipids, and weight.
- Address hypo/hyperglycemia and adjust insulin-to-carb ratios as needed.
- Interpret lab and blood glucose results to recommend changes in diet, medication, or behavior.
- Monitor and adjust medical nutrition therapy (MNT) and coordinate with exercise plans.
- Initiate and manage insulin pump therapy and Continuous Glucose Monitoring Systems (CGMS).
- Train patients on device use and interpret data for treatment adjustments.
- Work closely with physicians, nurse practitioners, and staff to coordinate care and communicate patient goals.
- Refer patients to other healthcare professionals as needed.
- Respond to patient inquiries via phone regarding glucose levels and nutrition.
- Maintain and update educational materials and resources.
- Document patient visits and complete billing forms accurately.
- Participate in DSMT Advisory Committee meetings and support program development.
- Adhere to clinic policies, HIPAA regulations, and billing procedures.
- Maintain at least 15 hours of diabetes-related continuing education annually.

Education and Experience Requirements:
Education:
- Minnesota RD License (required)
- Certified Diabetes Educator (CDE) certification (required)
Experience:
- Minimum 1 year working with diabetic patients in a clinic or hospital setting

- Travel independently throughout the clinic, office, or surgery center (which may include movement from floor to floor); frequent bending, lifting, standing, stooping, or sitting for long periods may be required.
