Job Summary:
The Registered Nurse in triage is responsible for assessing patient symptoms, determining urgency, and providing appropriate care guidance via phone or in-person. This role involves clinical decision-making to prioritize patient needs, offer health education, and coordinate timely referrals or follow-up with healthcare providers. The triage nurse acts as a critical link between patients and the healthcare team to ensure safe, efficient, and compassionate care.
Essential Job Functions:
- Advocate for patients’ rights, preferences, and needs, ensuring that they receive high-quality, compassionate care that is tailored to their individual circumstances
- Project a professional demeanor and appearance while maintaining the confidentiality of the patient, coworkers, and the hospital, as appropriate
- Provide high quality nursing care through patient assessment, diagnosis, outcome identification, planning, implementation and evaluation
- Conduct thorough patient assessments via phone or in-person to assess conditions and prioritize care
- Execute nursing interventions by collaborating with the interdisciplinary team
- Review and act on test results
- Review and manage communications for internal and external customers
- Use clinical judgment and established protocols to triage patients and determine appropriate level of care
- Escalate complex or high-risk cases promptly to the appropriate provider or department
- Assist and perform procedures (I.e. EKGs, dressing changes, injections)
- Provide support to providers during examinations and treatments
- Provide clear and accurate communication to patients on health information, education with discharge and self-care instructions and follow up care
- Assist with urgent patient care situations
- Ensure accurate care, collection, and handling of specimens
- Respond to life-saving situations based on nursing protocols and standards
- Document triage encounters and clinical findings accurately in the electronic health record (EHR)
- Coordinate referrals, follow-up appointments, or emergency care as needed based on patient condition
- Collaborate with physicians, and other healthcare staff to ensure continuity of care
- Respond promptly to patient inquiries, including medication questions, symptom management, and care instructions
- Report all occurrences in a timely manner
- Work independently within scope of practice to achieve clinical outcomes
- Maintain up-to-date knowledge of triage protocols, clinical guidelines, and regulatory standards
- Turnaround patient bays promptly after each visit per infection control standards
- Participate in quality improvement initiatives and staff training related to triage processes
- Ensure infection control protocols and safety standards are followed in all aspects of patient care
- Utilize the organization's secure messaging platform in accordance with departmental protocols, including, but not limited to, receipt and timely responses to communications related to on-call responsibilities

Knowledge, Skills, & Abilities:
- Medical Terminology
- Electronic Medical Records (EMR) systems
- Quality assurance and accreditation standards
- Understanding of HIPAA regulations, patient confidentiality laws and patient rights
- Effective English communication in verbal and written format
- Competent in conscious sedation according to hospital policy
- Quality work including attention to detail and accuracy
- Basic computer proficiency
- Carry out hospital customer service standards
- Prioritize tasks effectively through managing workload based on urgency and importance
- Capacity to work independently and as part of a multidisciplinary team
- Continuously learn and improve through staying up to date on job specific trends, policies, and new technologies
- Successfully demonstrate organization-wide performance review competencies
Qualifications:
- Demonstrates eligibility for employment in the U.S.
- Associate’s degree in nursing from an accredited school required
- Current RN license in the state of Wisconsin required
- BLS certification required upon hire
- Must obtain ACLS and PALS certification within six months of hire
- Triage experience required
- Experience in emergency room, urgent care, or ambulatory care setting preferred
- Must possess a cell phone that interfaces with the organization’s secure messaging system
