About the Role:

As a Nurse Practitioner at Forest Park Hospice, you will play a vital role in providing high-quality care, with a strong focus on hospice and pain management. Your core responsibilities will include performing comprehensive clinical assessments, developing individualized care plans, and ensuring patient safety throughout the continuum of care. You will also be responsible for conducting face to face visits and recertification in collaboration with the RN Case Manager. Working closely with a multidisciplinary team, you will help foster a collaborative environment aimed at improving patient outcomes. Your clinical expertise will be essential in managing complex medical conditions and delivering compassionate care to patients and their families. Your contributions will have a profound impact on the quality of life of those you serve, bringing meaning and comfort to their healthcare journey.  

Compensation:

This is an hourly position with a pay range of $55-$60 per hour, plus additional compensation for In-Person Face to Face visits.

Minimum Qualifications:

  • Master's degree in Nursing from an accredited program.
  • Current Nurse Practitioner certification and state licensure.
  • Experience in hospice care or pain management.

Preferred Qualifications:

  • Certification in palliative care or hospice nursing.
  • Previous experience working in a multidisciplinary team environment.

Responsibilities:

  • Conduct thorough clinical assessments of patients to determine their healthcare needs.
  • Develop and implement individualized care plans in collaboration with the healthcare team.
  • Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
  • Provide education and support to patients and their families regarding care options and pain management strategies.
  • Participate in team meetings and contribute to a positive company culture that emphasizes teamwork and collaboration.

Skills:

The required skills of clinical assessments and pain management will be utilized daily as you evaluate patient conditions and tailor treatment plans accordingly.
