About the Role:
The Physical Therapist PRN plays a critical role in delivering high-quality rehabilitative care to patients within their home environments, ensuring personalized treatment plans that promote recovery and enhance mobility. This position focuses on assessing patient needs, developing and implementing therapeutic interventions, and monitoring progress to achieve optimal functional outcomes. The role requires adaptability to varied home settings and collaboration with interdisciplinary teams to coordinate comprehensive care. The Physical Therapist PRN must maintain detailed documentation and communicate effectively with patients, families, and healthcare providers to support continuity of care. Ultimately, this position aims to improve patients' quality of life by restoring physical function and independence through expert therapeutic techniques.
Minimum Qualifications:
- Licensed Physical Therapist in the state of practice.
- Degree in Physical Therapy from an accredited program (Bachelor’s or Doctorate).
- Current CPR certification.
- Experience or training in home health physical therapy.
- Ability to pass background checks and meet health screening requirements.
Preferred Qualifications:
- Experience working in home health care settings preferred.
- Familiarity with electronic medical records (EMR) systems.
- Strong knowledge of geriatric rehabilitation and chronic disease management.
- Certification in specialized therapy techniques such as wound care or neurological rehabilitation.
- Excellent communication and interpersonal skills for patient and family education.
Responsibilities:
- Conduct thorough patient evaluations in home health settings to determine physical therapy needs and establish individualized treatment goals.
- Develop and implement evidence-based therapeutic interventions tailored to each patient's condition and home environment.
- Monitor patient progress regularly, adjusting treatment plans as necessary to ensure effective rehabilitation outcomes.
- Educate patients and their families on exercises, mobility techniques, and safety measures to support ongoing recovery and prevent injury.
- Collaborate with physicians, nurses, and other healthcare professionals to coordinate care and optimize patient outcomes.
- Maintain accurate and timely documentation of patient assessments, treatments, and progress notes in compliance with regulatory standards.
- Manage scheduling and caseload efficiently to provide flexible and responsive care as a PRN (pro re nata) therapist.
