This position requires trained professionals to be matched with 20 to 30 youth ages 16 and 17 in licensed foster care settings. Coaches meet young people “where they are” and build a trusting relationship with them using an authentic, trauma-informed, strength-based approach. In addition to providing emotional support, Coaches help young people make progress towards their academic, career development, housing/independent living, and other milestones and life goals. When a young person is unsure of their goals/interests, Coaches help them explore and expose them to potential opportunities. They then help young people connect to schools, programs, and opportunities in line with their goals/interests, persist in those settings, and plan/prepare for the next step along their journey. Coaches celebrate young people’s progress; if something doesn’t work out, Coaches help young people reflect and connect to the next opportunity.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
1. Build relationships with youth assigned on caseload. Coaches employ creative, out-of-the box approaches to engaging youth and do not give up if they are not responsive.
2. Provide minimal monthly contact for ongoing social/emotional support to youth served.
3. Build relationships and collaborate with the key adults in the young person’s life to support their goals and well-being, including other agency staff (e.g., Case Planners and Specialists), foster parents, school/program staff, and other significant adult figures.
4. Recognize, respect and respond to the unique, culturally-defined needs of all youth and young adults served.
5. Help young people explore their interests and expose them to opportunities in line with their interests.
6. Conduct Independent Living Skills Assessments using established tools.
7. Based on the outcome of the ILS Assessment scores, develop a training plan and a schedule of targeted life skills to develop competencies in identified areas.
8. Facilitate and coordinate individual and group life skills training events and activities for current youth in foster care.
9. Work 1:1 with young people to develop individualized plans that address academic and career development goals based on their interests/strengths and take measurable steps towards their goals.
10. Connect young people to quality schools, programs, resources, and opportunities in line with their goals.
11. Help young people persist in academic/career settings, navigate any challenges that arise, celebrate successes, and plan/prepare for the next step on their journey.
12. Should a youth not be successful in any particular school/program, Coaches help young people reflect on the experience and work with them to transition to the next opportunity.
13. Use an electronic platform each month to track contacts, young people’s goals, steps taken towards goals, and other key data.
14. Connect young people to peer group supports and build positive relationships.
15. Ensure young people who are aging out can access and maintain stable, affordable housing and build independent living skills.
16. Participate in case management planning activities such as other staffings and court hearings related to assigned youth.
17. Assist the youth with obtaining their State ID, Driver’s License, and establish a bank account.
18. Ensure the youth is educated about their rights and responsibility as a youth in foster care.
19. Ensure the youth is educated about Post 18 programs, requirements, and housing options.
20. Maintain program documentation on assessments, training and services specific to the youth in the statewide databased (FSFN) in accordance with rule/code/policy and procedure.

Qualifications


EDUCATION and/or EXPERIENCE
• A Bachelor’s degree in a Human Service discipline and 2+ years of experience working with young people

 

CERTIFICATES, LICENSES, REGISTRATIONS This position requires the following credentials.
• Possess a current Background Clearance Screening Letter.
• Possess a valid driver’s license and be insurable by the agency’s current insurance carrier if using a company automobile.
• Valid personal auto insurance
• Child Welfare Case Manager Certification
