MDS Assessor- Registered Nurse (RN)
Parker Jewish Institute for Health Care and Rehabilitation, located in New Hyde Park, NY, is seeking an experienced, reliable and highly motivated Clinical Assessor knowledgeable in MDS to join our dedicated team of professionals in Nursing-Clinical Operations.
In this key clinical role working alongside nursing leadership and the interdisciplinary care team, the Clinical Assessor is responsible for the implementation of Resident Assessments, using the MDS 3.0 and PRI, and PDPM. The Clinical Assessor will also conduct periodic reassessments of Medicare-eligible residents, participate in interdisciplinary team meetings, and serve as a resource for the staff regarding appropriate medical record documentation for MDS.

Position Qualifications
This position requires a current New York State RN license and three years of experience in a long-term care facility as an MDS Coordinator, along with knowledge of Medicare criteria and assessment regulations. The ideal candidate must possess excellent listening and communication skills, the ability to complete documentation in a timely manner, as well as attention to detail and accuracy when performing assessments.
