LOCATION: Dublin, Georgia
The Community Service Board of Middle Georgia's S.P.A.R.K. program is seeking a qualified individual to serve as our next Case Manager. The S.P.A.R.K. program is a peer-supported day services program for people with behavioral health diagnosis where they learn daily living skills, self-advocacy, and whole health recovery. The S.P.A.R.K. Case Manager will report to the Program Manager.
Responsibilities of the Case Manager:
- Thoroughly observes consumer's progress for compliance to and in obtaining treatment plan goals through one-on-one Interviews and other contact interactions.
- Writes concise, accurate and clear observation notes as required by departmental policy and according to American Psych System guidelines, and within policy timelines and with regard to DHR's documentation guidelines.
- Promptly notify appropriate professionals of any unusual individual incident/situation.
- Schedules follow-up appointments in a timely manner to ensure services are provided.
- Makes appropriate referrals for support service. Services may include but not limited to Vocational Rehabilitation, DFACS, SSA, Housing Authority, Senior Citizen's care, etc.
- Completes or monitors the completion of required forms within prescribed time frames.
- Meets with individual's continuity of care representative as required;
- Transport, or makes arrangements for transportation of individuals to appointments in a timely manner;
- Conducts individual sessions to provide appropriate guidance and direction pertaining to personal and social development within established guidelines.
- Conducts family counseling or educational sessions according to accepted methodologies.
- Adheres to ethical and professional counseling standards in all direct and indirect interactions with individuals, their families/significant others and other agencies.
- Conducts group counseling sessions using established procedures and methods to promote planned changes in individual's support system.
- Provides social and psychological support to individuals and their families through listening and discussing problems and progress as necessary.
- Recognizes the need for individual crisis intervention and reacts appropriately to routine situations and/or promptly notify person on call or immediate supervisor.
- All other duties as assigned

- High School Diploma/GED Equivalent
- Valid Georgia Driver’s License
- Effective verbal and written communication skills
- Strong interpersonal skills and the ability to work effectively with diverse communities
- Ability to work independently and in collaboration with others
- Experience with Microsoft 365 Office Products
- Ability to organize, prioritize and meet deadlines accordingly
Here’s what would put you over the top:
- Bachelor’s degree in a related field (psychology, social work, counseling, etc.)
- Previous experience in case management or behavioral health
