Position Description:  Supports youth in PPCH Foster Care, foster families, and those involved in the foster care system by facilitating successful foster family partnerships and settings. Acts as case manager and liaison for children, foster families, and those committed to helping youth in foster care reach their full potential.

 

Essential Duties: 

  1. Ensures that the self-expression, self-worth, and well-being of youth in PPCH Foster Care is supported and respected.  
    • Promotes collaboration with youth in Foster Care, their loved ones, Foster Families, regulatory agencies, and support team. 
    • Ensures that training, resources, and assistance are provided to Foster Families and others so they may create a safe and supportive home for youth in Foster Care. 
    • Collaborates and attends meetings with foster families, schools, treatment teams, counties, and others as a representative of PPCH Foster Care. 
    • Continually evaluates and assesses the programs’ strengths and weaknesses to improve services and supports using a person-centered approach. 
  2. Supports a culture where children and others in services are healthy, safe, and valued by others. 
    • Models and teaches respect for others, including youth in Foster Care, Foster Families, guardians and loved ones, and team members. 
    • Ensures the safety and well-being of youth in PPCH Foster Care by providing oversight, guidance, and resources to Foster Families.
    • Monitors medical services, appointment attendance and follow-up for youth in PPCH Foster Care.  
    • Ensures that the preferences and needs of youth in PPCH Foster Care are considered.
    • Conducts required monitoring, environmental studies, and home visits and ensures prompt follow-up when deficiencies or areas for improvement are found. 
  3. Responsibility for compliance with the Colorado Rules and Regulations for Child Placement Agencies, PPCH policies, and the rules and regulations of other oversight agencies.         
    • Oversight of and responsibility for foster care tracking and documentation, such as ensuring bookkeeping, reporting, auditing, and recordkeeping meet the standards of PPCH and State and Federal agencies. 
    • Ensures foster families and providers have completed required trainings, including annual refreshers, according to established timelines. 
    • Maintains knowledge and understanding of PPCH and regulating agencies’ policies, procedures, rules, regulations, and guidelines. 

Other Duties:

  1. May provide oversight or supervision for a PPCH Foster Care paraprofessional or trainee. 
  2. May assist with administrative duties.
  3. Other duties as assigned. 


Qualifications:

Education and Training: 

  • Must possess either a Bachelor’s degree in the social or behavioral sciences, OR
  • Bachelor’s degree in a non-related field plus 2 years’ experience supervised by an individual with a master’s degree in social or behavioral sciences.   
  • Successful completion of all required PPCH training

Experience: 

  • 2 years of experience in social services or related field is preferred.  
  • 2 years of experience working with individuals with disabilities is preferred. 

Knowledge, Skills, and Abilities: 

  • Excellent written and verbal communication skills, with effective listening skills, even when non-traditional communication is used. 
  • Ability to collaborate with or manage others. 
  • Strong organizational and time management skills. 
  • Ability to respectfully address significant issues of health and safety with foster parents and teams. 
  • Knowledge of foster care in Colorado and Volume 7 of Colorado State Rules and Regulations preferred.   
  • Skill and ability to constructively receive and provide feedback – Able to have difficult conversations. 
  • Valid driver’s license and reliable transportation with the ability to meet PPCH’s driving requirements. 

 

Physical Requirements and Working Conditions:

  • Lifting and carrying of approximately 15 lbs and may be required to bend, stoop, squat as it pertains to assessing an environment. 
  • Work is mainly performed in private homes or in the community using a laptop and/or mobile device and is seldom performed in a typical office environment. 
  • Regular and frequent driving throughout the Pueblo and Colorado Springs areas is required.  Occasional travel to the main PPCH office in Lakewood may be required.
  • Daily phone and face-to-face contact with people including those receiving services, family and community members, and professionals in the community.
