What You Will Do:
- The RN is responsible for the nursing care of persons on his/her caseload. This includes updating the Individualized Treatment Plan (ITP) when the patient is receiving medication services only. Nursing staff will provide care management for patients whose chronic health conditions effect behavioral health conditions. A team player who is flexible and able to listen to input from participants and other team members. Provides patient-centered care in a manner that conveys acceptance, instills hope, and reduces stigma.

- Performs tasks necessary to support revenue including timely and accurate documentation within departmental guidelines.
- Provides care management such as obtaining information from other healthcare providers as needed, conducting medication reconciliation on a routine basis to ensure the record reflects all medications prescribed for/used by the patient, providing education regarding physical and behavioral health conditions when indicated and within the nurse’s scope of practice; and referring patients to additional behavioral health services when medication services alone are not sufficient for stabilizing symptoms.
- Provides education and interventions to support medication adherence.
- Alerts the prescriber to concerns regarding changes in physical health status, medications, behavioral health symptoms and/or medication side effects, and lab results indicating critical values.
- Performs general nursing duties including taking vitals, drawing blood, collecting urine samples, etc.
- Is responsible to ensure all medications are stored, tracked and disposed of per departmental policy. This includes timely completion of reports due to the Pharmacy Department.
- Understands the role of Patient Care Standards (PCS) as a measure of our commitment to serving those in need and ensuring that access to care is provided. Ensures PCS standards are met and actively utilizes and responds to strategies provided through supervision when there are challenges in meeting these standards.
- Is able to effectively and efficiently utilize EMR and video-conferencing software and any other technology as required for patient care. Utilizes work email on a daily basis keeping current with FRMC and departmental communications. For new hires, these skills are in place by the end of the probationary period. Ensures all electronic forms of communication, that include PHI, are compliant with federal laws regarding confidentiality: HIPAA and 42 CFR, Part 2 laws regarding confidentiality.
- When tele-services are provided, nursing services are adapted to ensure the same standards of patient care are provided.
What You Will Need:
- RN with at least 3 years of experience in healthcare setting. Knowledge of chronic physical conditions. Behavioral health experience strongly preferred. Holds valid Ohio professional license as appropriate.
- Experience working with individuals who have a mental, physical or substance use disorders is preferred.
Ability to be mobile and possess a valid State of Ohio motor vehicle operator’s license and be insurable under Firelands’ auto policy. Employee must provide proof of personal automobile liability insurance coverage upon request.
Successful completion of a 90-day probationary period.
