Tabitha's Hospice in Grand Island, NE is seeking a Social Worker to work 8a-5p Monday -Friday with an every 5th night and every 5th weekend on-call. This schedule includes a weekend and holiday rotation.
The Social Worker is responsible for providing direct social services to Seniors/residents/clients and family members to ensure that the medically related emotional and social needs of the Senior/client are met or maintained. This position is also responsible for needs assessment, care planning, problem resolution, discharge planning and client advocacy, and completion of documentation per State and Federal legislations. The Social Worker embraces all components of the job, fostering an environment that allows Tabitha to provide optimum care to Tabitha’s Seniors/clients.
Essential Functions
- Implements the social service program to meet the medically related emotional and social needs of the Seniors/clients and the family.
- Assists Seniors/clients with problem solving, provides crisis counseling, bereavement counseling, information resources and referrals as appropriate.
- Participates in discharge planning, completes Senior/client assessments and development and implementation of care plans for each Senior/client on case load.
- Develops working relationships with Seniors/clients, families, and staff for further understanding of significant social and emotional needs underlying Senior’s/client’s health problem.
- Serves as a liaison and an advocate for the Senior/client and their family.
- Provides information to Senior/client/families regarding financial concerns, including general information regarding Medicare, Medicare Part D, and application for Medicaid and other financial assistance programs.
- Provides Senior/client contacts to assist with the development and/or maintenance of positive self-image and promotes continued growth of each Senior’s/client’s potential.
- Completes assessments and documentation as required by state/federal regulatory requirements, as well as department policy
Ideal Candidate
- EDUCATION: Bachelor’s or Master’s Degree in human services related field required. Graduate of an accredited school of social work preferred.
- EXPERIENCE: One year experience working with families in crises required. Minimum of two years’ experience working closely with Seniors/clients and their family members at Tabitha is acceptable. Nursing home and hospice experience preferred.
- CREDENTIALS: Must maintain current licensure or certification, as appropriate. Must meet Tabitha’s vaccination requirements.
- COMMUNICATION: Must be able to speak, read, write, and understand the English language as well as terminology and jargon specific to the job. Sufficient fluency to be able to understand and follow written and verbal instructions, provide instructions to others, read and understand policies, procedures or other similar documentation, interact successfully with Seniors/clients/caregivers, hold a conversation, conduct or participate in meetings, compose professional e-mails or other written communication, prepare legible, accurate and thorough documentation as required by the job and effectively interact with coworkers, customers, vendors, volunteers, managers and family members. Ability to recognize and interpret non-verbal cues. Ability to respond calmly and clearly to emergencies and emotional situations.
- WORKSITE CONDITIONS: Works indoors in a facility maintained by Tabitha that will typically be in good repair, clean, well-ventilated and well lit. Work is performed in a standard office setting and in Tabitha Living Communities. Works in hospital and other care facility environments. Frequently seated at a desk, working at a computer for extended periods. Works under daily and hourly deadlines, requiring uninterrupted periods of intense activity. Adapts to changing workload and work environment. Traveling is required to home environments and community events. Frequent exposure to interruptions and changes in priorities. Works beyond normal scheduled hours and on weekends and holidays when assigned. Is subject to extended hours or callback during emergency conditions or based upon needs of the organization.
- PHYSICAL REQUIREMENTS: Ability to gain access to the work area and then safely perform the duties of the job while utilizing standard and specialized equipment, including but not limited to the equipment described below. Ability to move about the work area. Frequent periods of intense activity occasional need for physical strength or physical exertion. Must have the ability to spot errors or to sense when data output contains an error. Vision and hearing within normal limits (with or without correction). Must have unrestricted lift/push/pull ability and pass a 25 - pound physical exam demonstrating strength and agility prior to employment and from time to time thereafter at Tabitha’s discretion.
- EQUIPMENT USAGE: Standard and specialized equipment including laptop, PC, tablet, photocopier, scanner, fax machine, and telephone.
