Schedule: Saturday & Sunday, 8:00 AM – 5:00 PM
Location: Hybrid – remote triage with homecare visits as needed

About the Role
Mountain Valley Hospice & Palliative Care is seeking a Licensed Practical Nurse (LPN) to provide compassionate weekend support for our clinical team. This role ensures patients and families receive timely, high-quality care on Saturdays and Sundays while supporting continuity across our hospice program.
Key Responsibilities
- Respond to patient and family needs primarily by phone.
- Assign and coordinate patient visits to ensure adequate coverage.
- Complete homecare visits as needed to support patients and families.
- Collaborate with the interdisciplinary team to maintain seamless patient care.
Qualifications
- Current LPN license.
- Hospice or home health experience preferred.
- Strong communication and organizational skills.
- Compassionate, dependable, and adaptable in a fast-paced environment.
