About the Role:
The PRN Hospice Licensed Practical Nurse (LPN) with weekend availability plays a critical role in delivering compassionate, patient-centered care to individuals facing life-limiting illnesses. This position focuses on providing skilled nursing care in a hospice setting, ensuring comfort and dignity for patients during their end-of-life journey. The LPN will collaborate closely with a multidisciplinary team to develop and implement individualized care plans that address physical, emotional, and psychosocial needs. Weekend availability is essential to maintain continuity of care and support for patients and their families during times when regular staff may be limited. Ultimately, this role contributes significantly to enhancing the quality of life for patients while supporting their families through education and empathetic communication.
Minimum Qualifications:
- Current and valid Licensed Practical Nurse (LPN) license in the state of practice.
- Experience working in hospice, palliative care, or a related healthcare setting.
- Ability to work weekends as required by the PRN schedule.
- Strong communication and interpersonal skills to effectively interact with patients, families, and multidisciplinary team members.
- Basic proficiency in electronic health records and documentation.
Preferred Qualifications:
- Certification in Hospice and Palliative Nursing (CHPN) or equivalent specialty certification.
- Previous experience working within a multidisciplinary hospice care team.
- Training in pain and symptom management specific to end-of-life care.
- Familiarity with community resources and support services for hospice patients and families.
- Demonstrated ability to provide culturally sensitive care.
Responsibilities:
- Provide direct nursing care to hospice patients, including medication administration, symptom management, and monitoring patient status.
- Collaborate with physicians, registered nurses, social workers, chaplains, and other team members to develop and update comprehensive care plans.
- Conduct patient and family education regarding disease progression, medication use, and comfort measures.
- Document all nursing interventions, patient responses, and changes in condition accurately and timely in compliance with regulatory standards.
- Respond promptly to patient and family needs during weekend shifts, ensuring continuity and quality of care.
