What will you do as an Access Center Coordinator
The Access Center Coordinator provides timely, and responsive customer- centered service to referral sources, patients, patient families, and others contacting HopeHealth regarding our services. Under the direction of the Access Center leadership, this role is responsible for the timely and accurate processing of palliative referrals, hospice referrals and hospice admissions. The ideal candidate is passionate about end-of-life care; can effectively multi-task and prioritize; is highly collaborative and has a strong work ethic.
This is a full-time position, 40 hours per week and is eligible for benefits the hours are 10:00am-6:30pm
What will you do as an Access Center Coordinator
· Understands the hospice benefit and eligibility and where services are provided.
· Familiar with HopeHealth palliative care programs, their benefits and patient eligibility.
· Provides first line customer service by responding to customer calls. Provides information to callers and/or routes calls to appropriate personnel, takes messages or relates information in a confidential, accurate, timely and professional manner.
· Enters and process patient referrals in a timely, complete, and accurate manner.
· Serves as a liaison between patients, the caregiving staff, and the billing department, supporting all three with comprehensive chart management and communications to ensure seamless patient care.
· Works closely with Palliative Provider Services to ensure that referrals are complete and accurate and obtains any additional information needed to process the referral.
· Collaborates with the Revenue Cycle team to ensure timely and accurate payer information is entered into the Electronic Health Record.
· Understands hospice reimbursement and the different levels of hospice care.
· Communicates with team members, patients, and their families regarding the status of DME supplies ordered.
· Communicates all pertinent patient status information to appropriate community physicians and other referral sources.
· Dispatches information and coordinates visit schedules for visiting pre-admission staff.
· Is familiar and is able to articulate all services offered by HopeHealth including hospice and palliative care; grief and caregiver support; educational programing and community support.
· Consistently applies HopeHealth values in all aspects of work, complying with all established HopeHealth policies and procedures including quality assurance, safety, infection control and HIPAA.
· Completes other duties as requested.
· Participates in weekend and holiday rotations with other Access Center coordinators.

Qualifications for this job as an Access Center Coordinator
· To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
· High school diploma plus additional specialized training.
· Associate degree, medical assistant and/or medical secretary background preferred.
· Two years of related experience. Comfortable working in a collaborative and fast paced environment.
· Ability to read and comprehend instructions and correspondence.
· Ability to clearly present information effectively to varying levels of staff and management physicians, patients, administrative assistants, operations managers and supervisors and other support and professional staff. In addition, the ability to read medical terminology is required in the translation of orders from verbal mode to written modes.
· Ability to apply mathematical concepts such as fractions, percentages, ratios, and proportions to practical situations.
· Ability to evaluate situations according to established standards and policies.
· In addition, ability to define problems, collect data, establish facts, and draw valid conclusions.
· Ability to interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
· Proficient skills using a computer and ability to effectively be trained further in computer skills for other job-related tasks.
