Capstone Hospice is seeking a dedicated and empathetic Clinical Liaison to join the dedicated admissions team. This position is responsible for facilitating the transition of patients from the hospital care (surrounding Metro Atlanta area hospitals) to home hospice services, ensuring that they receive comprehensive and compassionate end of life care. This role involves coordinating with healthcare providers, patients, and families to provide a seamless transition and support throughout the hospice care process.
As a Clinical Liaison your presence and services form a first impression for patients and families facing terminal illness and should convey comfort and assurance to all involved. For this role, you need to be a registered nurse who sees the value of hospice care in alleviating pain and suffering. A compassionate nature and excellent interpersonal skills will make you a good candidate for this position. We expect you to be knowledgeable in communicating hospice to patients and their families, caring for the terminally ill and performing tasks such as administering medication and providing emotional support. The goal is to help patients live their final days with dignity and comfort.
This position has the expectations of working Monday - Friday, 9:00am-6:00pm
Essential Responsibilities:
Patient Transition Coordination:
- Manage the transition process for patients moving from hospital care to hospice services, ensuring that all aspects of care are addressed.
- Collaborate with hospital staff to identify patients who may benefit from hospice care and initiate referrals.
- Coordinate with hospice and hospital teams to develop and implement personalized care plans for patients.
Communication and Liaison:
- Serve as the primary point of contact between the hospital, hospice teams, patients, and their families.
- Facilitate clear and compassionate communication regarding patient care plans, goals, and hospice services.
- Address any questions or concerns from patients and families about the transition to hospice care.
Education and Support:
- Offer emotional and informational support to patients and families throughout the hospice care process.
- Provide support to hospital and hospice staff on best practices for managing end-of-life care and transitions.
Quality and Compliance:
- Ensure that the transition process complies with regulatory requirements and hospice care standards.
- Monito patient outcomes and satisfaction to identify areas for improvement in the transition process.
Administrative Duties:
- Maintain accurate and confidential documentations related to patient transitions and hospice care coordination.
- Stay informed about updated in hospice care regulations and best practices.
Requirements
Candidate must meet the following requirements to be considered:
- Nursing degree with current active Georgia nursing license as a Registered Nurse (RN)
- Minimum of 3 years’ experience in a clinical setting, with at least 1-2 years in hospice or palliative care.
- Certification in hospice and palliative care, a plus.
- Current CPR Certification
- Experience & proficiency in EMR/EHR system.
- Formal experience in a direct hospice care setting such as position as a Hospice Admissions or Hospice RN Case Manager or Hospice Visit Nurse.
Candidates should possess the following knowledge, skills and abilities to be successful in this role:
- Proven experience as hospice nurse/registered nurse.
- Knowledge of nursing care processes and medication guidelines/side-effects
- Strong nursing assessment skills
- Committed to safety precautions and high ethical standards.
- Proficient in English (understanding and articulation)
- Sound judgement and attention to detail
- Excellent communication skills and a compassionate nature
- Cool-tempered and patient
- Organized and efficient, strong time-management
- Emotional/mental stability and stamina
Work environment:
- Fast paced hospital environment with interaction with hospice services and patient families.
- Must be able to handle multiple tasks and manage priorities effectively.
- Manage patient care within a hospital setting.
