STATEMENT OF PURPOSE

The Home Health Clinical Coordinator provides care coordination for home health services and is responsible for the day-to-day case coordination of the home health agency. The clinical coordinator is responsible for planning, coordinating, and delivery of care of home health patients, as well as quality review of documentation and OASIS data. The clinical coordinator works closely with the Home Health Manager.


  $20,000 SIGN ON BONUS 


This is a full time position working 72 hours per two week pay period. Days of work are Monday - Friday from 8:00am - 4:30pm with one weeknight call every fourth week and every ninth weekend. 


JOB FUNCTIONS

  • Evaluates all referrals for eligibility for home health services, contacts insurance for authorization, and provides education to patients and families regarding home health services upon referral.
  • Collaborates with the primary nursing team in the identification of physical, psychological, social, and spiritual needs of home health patients and oversees the development of a care plan to meet these needs.
  • Assures the appropriate use of the nursing process in the delivery of care to all ages of patients including geriatric, adult, adolescent, and pediatrics.
  • Supervises and participates in the planning, coordinating, and delivery of patient care; assists primary care staff to implement treatment per physician plan of care to meet these needs.
  • Scheduled in the Hospice/Home Health call rotation consisting of nights, weekends and holidays.
  • Manages daily patient visit schedule and coordinates with RNs and aides to meet physician-ordered visit frequency.
  • Participates in daily discharge planning with JRMC PCU and social services team.
  • Assists Home Health Manager in ensuring agency compliance with regulatory standards. 
  • Assists Billing Specialist in compiling home health agency statistics monthly.
  • Responsible for quality review work queues within electronic medical record.
  • Reviews OASIS datasets/nursing and therapy assessments at each transition of care.  
  • Supervises the maintenance of medical records, assuring accuracy, confidentiality, and completeness of the chart, and performs audits of open and closed charts quarterly.
  • Supervises and maintains confidentiality of patient's illness, history, financial status, and family situation.
  • Provides feedback and education regarding quality and completeness of documentation to staff.
  • Identifies need for staff education and assists in the development of inservice programs for home health staff.
  • Participates in staff selection and coordinates orientation of new personnel.
  • Performs other related duties as assigned or requested.


Requirements

QUALIFICATIONS/REQUIREMENTS FOR THIS POSITION: (HOME HEALTH CLINICAL COORDINATOR-RN)

  1. PREPARATION AND TRAINING - Work requires a professional level of knowledge acquired by completing an accredited RN nursing education program. Must hold RN licensure from the North Dakota State Board of Nursing for the current year. Prefer preparation at BS level. Position requires completion of BLS CPR. Position is required to attend Crisis Prevention Intervention (CPI) training in the appropriate timeframe according to department orientation checklist.
  2. WORK EXPERIENCE - Requires 3-5 years general Medical/Surgical nursing experience. Requires two years Home Health/Hospice experience. 
  3. ATTENDANCE - Punctual and regular attendance is an essential responsibility of each employee at JRMC. Employee are expected to report to work as scheduled, on time and prepared to start working. Employees are also expected to remain at work for their entire work schedule. 
  4. ANALYTICAL ABILITY - A high level of analytical ability is required in a variety of duties in order to gather and interpret complex data and to find solutions to difficult problems.
  5. INDEPENDENT JUDGEMENT - Independent nursing judgement is often required by the professional RN based on established policies and procedures.
  6. CONTACTS WITH OTHERS - Considerable contact with physicians and personnel from other departments and residents of area served.  
  7. SUPERVISING THE WORK OF OTHERS - May be required to plan, assign work to, and check the work of nursing personnel doing related work on the shift. Supervision is provided to LPNs, Nurse Techs, Nurse Aides, and Unit Clerks.
  8. RESPONSIBILITY FOR WELFARE OF OTHERS - It is the responsibility of the professional RN to continuously uphold the highest degree of performance within currently defined and acceptable nursing practice standards.
  9. MENTAL/VISUAL EFFORT - Auditory and visual acuity necessary for reading charts and working with medical devices; color perception necessary for interpreting testing procedures involving color comparisons.
  10. WORKING CONDITIONS - OSHA job classification defined as Category I in which the employee does have exposure to bloodborne pathogens. Exposure to patients with infectious diseases and unsanitary conditions within patient's residence. Participates in and complies with JRMC Safety Management program. Work may be beyond usual working hours, on weekends, and holidays to respond to patient-family needs.  
  11. OTHER - Position requires the individual to provide a vehicle for transportation to and from home visits. Must hold a current unencumbered North Dakota Driver's license and appropriate automobile insurance. Position requires extensive travel by car.
  12. BACKGROUND CHECK - Background check is required for this position.
