The Opportunity 

We’re seeking a dedicated Clinical Pharmacist to join our Comprehensive Care team within Advanced Primary Care (APC). In this role, you will work alongside primary care providers, nurses, and care coordinators to optimize medication therapy and improve health outcomes for high-risk, complex patient populations. This ambulatory care position offers the opportunity to make a meaningful impact through comprehensive pharmaceutical care in a collaborative, value-based healthcare environment.

Your Impact 

Patient Care Management

  • Manage a designated panel of high-risk patients by developing and maintaining goal-driven care plans
  • Conduct comprehensive medication therapy management (MTM) reviews for complex patients
  • Perform medication reconciliations post-discharge and support transitional care efforts to reduce readmissions
  • Monitor patient adherence, identify barriers, and implement strategies to improve adherence (e.g., patient education, personalized medication management support, cost-lowering options)
  • Optimize medication regimens to enhance therapeutic outcomes and reduce adverse drug events

Care Coordination & Collaboration

  • Partner closely with primary care providers and patients to identify barriers to appropriate disease management and implement care plans
  • Serve as a clinical resource to providers, offering evidence-based recommendations to support guideline-driven care
  • Collaborate with the care team to address both clinical and non-clinical needs impacting patient outcomes

Education & Support

  • Provide patients with education on disease state management and safe, effective medication use
  • Offer guidance to providers on therapeutic alternatives, medication protocols, and evidence-based practices
  • Contribute to provider and care team learning on medication therapy and chronic disease management

Documentation & Quality

  • Maintain accurate and timely documentation of patient encounters, MTM reviews, and care plans
  • Track outcomes and pharmacy-related quality measures to support population health goals
  • Participate in team meetings, case conferences, and quality improvement initiatives
  • Ensure compliance with organizational policies, procedures, and regulatory requirements

Who You Are 

Education & Licensure

  • Required: Doctor of Pharmacy (PharmD) from an accredited program
  • Current pharmacist license in Maine, New Hampshire, and Massachusetts (or licensed in one with willingness to obtain others)

Experience

  • 2–3 years of clinical pharmacy experience in ambulatory care, primary care, or a related setting
  • Experience delivering medication therapy management and chronic disease management
  • Demonstrated experience working with high-risk, complex patient populations
  • Strong knowledge of chronic disease states (e.g., diabetes, hypertension, heart failure, COPD)
  • Proficiency with medication adherence strategies and behavioral interventions

Skills & Competencies

  • Strong clinical judgment and problem-solving skills
  • Excellent communication and interpersonal abilities for patient and provider collaboration
  • Ability to manage multiple priorities while maintaining patient-centered focus
  • Proficiency in electronic health records and clinical technology platforms
  • Commitment to cultural competency and addressing health equity

Bonus Qualifications 

  • Completion of PGY-1 and/or PGY-2 residency in ambulatory care pharmacy
  • Board Certification in Ambulatory Care Pharmacy (BCACP) or Pharmacotherapy (BCPS)
  • Additional certifications (e.g., MTM, diabetes education)
  • Experience with collaborative practice agreements and provider partnerships
  • Knowledge of healthcare quality metrics and value-based care models
  • Familiarity with social determinants of health and health equity initiatives
