Position Duties

Under broad supervision, the Housing Case Manager performs tasks necessary to provide rental assistance and housing support to qualified participants through the HOPWA (Housing Opportunities for Persons with HIV/AIDS) grant.

Responsibilities include conducting tenant case management, performing housing unit inspections for compliance, facilitating briefing and training sessions for participants and landlords, and maintaining accurate and timely documentation. Case management activities involve face-to-face visits, goal setting, and conducting annual assessments to support housing stability and improve the overall well-being of clients.

This role plays a critical part in ensuring that eligible individuals receive and maintain stable housing while working toward greater self-sufficiency and health outcomes.

Primary Duties:
  • Conduct intakes/assessments to assess client needs to determine appropriate type of housing.
  • Assist clients in developing and implementing individualized housing plans.
  • Facilitate the placement of clients into appropriate housing, working closely with client, building managers, and/or landlords.
  • Provide case management expertise to include, but not limited to, assessments, treatment planning, supportive counseling, linking to community resources, monitoring progress, collaboration with other providers, and care coordination.
  • Facilitate meetings with clients, landlords, case managers, and/or other service providers, as needed.
  • Travel to meet with clients and maintain home visit schedules with clients per the HOPWA guidelines.  Maintain at least monthly contact with assigned clients.
  • Ensure that clients and landlords are aware of rules and regulations specific to the housing programs.
  • Complete training for and become proficient in HMIS (Homeless Management Information System), the client tracking software application.
  • Complete required documentation and maintain case files.   This will include Client Track case notes, verifying employment and income benefits, and certifying rental share amounts.
  • Collaborate with other departments and outside partners to ensure team-oriented, client-centered focus.
  • Participates in scheduled staff meetings, supervision meetings, and other required meetings

Minimum Qualifications

Master's degree in a social services field from an accredited college or university OR Bachelor's degree from an accredited college or university AND one (1) year of related experience OR Associate's degree from an accredited college or university AND three (3) years of related experience OR four (4) years of related experience.

Preferred Qualifications:

  • Minimum of a Bachelor's is preferred.
