Dominion Care is seeking a dedicated Hybrid Direct Support Professional/Service Coordinator (DSP/SC) to provide direct, hands-on support to individuals while also overseeing service coordination and care planning. In this dual role, you'll work closely with individuals to support their daily living skills and community integration, while also ensuring services are person-centered, compliant, and of the highest quality. SUMMARY/OBJECTIVE:
Assist and support individuals to maintain overall health, develop skills in activities of daily living, identify, access, and engage in their community and home environments, facilitate relationship building with greater community. Oversee service delivery, care coordination, and documentation for identified caseload. Continue growth and learning in professional development, ensuring quality and timely completion of progress notes for Medicaid billing, completion of all daily, monthly, quarterly, and annual required assessments, plans, and authorizations. Maintain quality through audit and peer review, and self-assessment. Maintain integrity and represent TRACK core competencies. Ensure individuals initiative, autonomy, and independence in making life choices regarding daily activities, physical environment, community integration and participation, and other life choices are optimized in accordance with HCBS training.
PREFERRED EDUCATION and EXPERIENCE:

A passion for supporting and encouraging individuals diagnosed with intellectual or

developmental disabilities.

Experience as a Direct Support Professional in the Human Services Field for 1+ years; currently
-
working as a DSP
-
Experience with professional writing
-
Willingness to learn and grow in position as a DSP
-
Valid driver's license

Desire to make a difference
ESSENTIAL DUTIES and RESPONSIBILITIES:
-
Coordinates the development and implementation of service plans, objectives and methods necessary for providing effective services to individuals.
-
Complete quarterlies for individuals served to summarize the service provided.
-
Acts as liaison with providers for process reporting and service planning.
-
Assists in admission process and discharge planning.
-
Participates in provider meetings, community meetings and other providers and program activities essential for the operation of an effective program.
-
Supervise staff members assigned to clients and encourage continuous quality improvements in care and corresponding documentation.
-
Demonstrates effective and professional communication with persons served, their family and support systems, as well as other members of their team.
-
Facilitates opportunities for integration into the greater community through open communication with persons served to learn individual preferences and barriers and connecting individual(s) with community resources related to those interests.

Optimizes, but does not regiment, individual initiative, autonomy, and independence in making life choices regarding daily activities, physical environment community integration and participation, and other life choices.

Train and assist in functional skills related to personal care and ADLs (completing personal care tasks when physically unable to do so, ensure hygiene and eating needs are met, complete daily tasks. Including using the restroom, bathing, eating, grooming).

Trains in functional skills related to use of community resources (transportation, shopping, social and recreational activities, etc.).

Monitors health and physical condition of people served. If medication management certification is completed, person may assist with administering medications and keeping an accurate and timely Medication Administration Record.

Always ensures health and safety of individuals.

Facilitates behavioral supports as appropriate; provides training in adaptive behavior for home and community in accordance with training provided for behavior management (redirecting anger, handling social encounters with others, developing a circle of support).

Facilitates active involvement of persons served and families/support systems in the service delivery process; maintain knowledge of individuals’ daily goals and turns in completed documentation on achieving those daily goals.

Maintains accurate time by clocking in and out at the start and end of shift.

Maintains current personal car insurance.
