Willow Valley Communities is hiring a Healthcare Social Worker for one of our skilled care buildings! The Healthcare Social Worker will function as a member of the inter-disciplinary team that is responsible for planning and implementing the care of each Resident on their caseload, will complete MDS documentation, assess the psycho-social needs and plan interventions to meet the Residents' needs, and be a Resident advocate.

Wage:
- $23/hour+
Responsibilities:
- Initiates Resident contact, interviewing and counseling for the proper assessment of care.
- Listens and responds to Resident and family member's concerns, providing counseling regarding disease progression and directing them to
- appropriate resources.
- Coordinates plan of care for Residents upon admission and updates quarterly, annually and with significant change in condition. Invites Resident's family as needed.
- Establishes care plans/support plans as related to Social Services, reviews and updates as needed.
- Provides oversight of the Resident grievance procedure.
- Responds to Resident and family needs/concerns, and assists with resolution.
- Addresses financial concerns of Resident and families, referring to business office as needed.
- Participates in care plan/support plan meetings for Resident/family.
- Coordinates resources to promote highest level of functioning and facilitates discharge to appropriate level of care.
- Collaborates with Transition Coordinator.
- Acts as contact/liaison between facility and other community organizations/hospitals.
- Coaches Team Members through Resident concerns.
- Responsible for admission and discharge process, completing admission notification and paperwork.
- Facilitates the onboarding meeting in skilled care.
- Coordinates additional Resident services as indicated and/or requested.
- Maintains proper documentation pertinent to discharge planning, social histories and Department of Health and/or Department of Human Services regulations.
- Responsible for coordinating the Behavior Management program and the Psychotropic Medication Review meeting (in skilled care).
- Coordinates services of the psychiatrist, psychologist, eye and dental visits.
- Provides education to new Team Members, Residents and Residents families on Resident rights, confidentiality and role of Social Worker.
- Attends/Town Meeting, consisting of Residents, families and volunteers.
- Completes and update face sheets as needed.
- Participates in Continuing Care Assessment Team Meetings (CCAT).
- Completes cognitive and Level of Living Assessments as needed.
- Advocates for Resident Rights and makes referrals as needed.
- Provides data to the Quality Improvement Committee and tracks and reports on quality indicators assigned to social services, when necessary.
- In skilled Care completes MDS sections which pertain to Social Services, and completes appropriate documentation.
- Provides support with end-of-life issues for Residents and families.
- Coordinates appropriate Resident placement in Supportive Living.
- Completes Quality Indicators as assigned to Social Services.
- In skilled care educates family/Resident on Medicare coverage, completes Medicare denial/cut letters when appropriate, and attends weekly Medicare meetings.
- Develops and maintains objectives and standards, assuring Residents a clean, comfortable, orderly and safe environment.
Qualifications:
- Must have a bachelor's degree in social work or a bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology.
- One year of supervised social work experience in a health care setting working directly with individuals.

- 40 hours/week

- 8:00am-4:30pm Monday-Friday
