WHAT YOU’LL BE DOING
Frontier Direct Care (FDC) Nurse Manager (NM) has a wide range of responsibilities. The NM plays a crucial part in ensuring that the practice provides exceptionally thorough member care. Additionally, the NM ensures that Medical Assistants (MA) make relentless progress in their ability to care for members in an elevated way. Key duties include:
MA Medical and Clinical Operations Oversight: The NM manages MAs to ensure efficient and effective practice operations.
- Support MAs in performance of triage, delegated duties and patient care
- Manage communications directed to practice: email, fax, phone
- Vaccine supply and storage coordination
- Oversee practice inventory and supply
- Oversee practice billing reconciliation
Population health and Chronic Disease Management: The NM researches, plans and coordinates interventions designed to improve practice population health.
- Monitor practice-wide data related to targeted high-impact pathologies and metrics
- Develop and implement population health interventions in collaboration with the Medical Director
- Coordinate individualized education, monitoring, and health coaching for high-risk members in collaboration with Medical Assistants
Complex Case Coordination
- Collaborates with Direct Pay staff, Medical Director and Primary Care Providers to identify members with complex care needs and to establish individualized plans of care.
- Facilitates care coordination meetings between multilevel Direct Care and Direct Pay teams to minimize care delivery delays and maximize member success
Provides direct and indirect coordination of member’s complex needs by
- Scheduling and follow-up care and ensuring member attendance
- Monitoring treatment adherence
- Facilitating communication between providers
- Coordinating medication refills and access to Durable Medical Equipment (DME)
MA Management: The NM is responsible for hiring, training, and evaluating MAs. They also ensure that all staff adhere to strict safety standards and compliance.

- Ongoing MA Training and annual competency assessments
- Support MA with Interpersonal Conflict Resolution
- MA PTO/schedule coordination
- MA 1:1
- MA Mentor
Regulatory Compliance: The NM ensures that all healthcare regulations and safety standards are applied.
- CLIA
- Medical Records
- BLS - maintain certification to train other staff
Liaison Role: The NM acts as a liaison between the MA and Lead primary care provider (LPCP).
Requirements
Qualifications
- Active and unrestricted LVN license in Texas.
- Minimum 5 years of clinical LVN experience (clinic setting preferred).
- Minimum 2 years of people management experience, ideally supervising Medical Assistants.
- Experience working in a family, primary care or other clinic setting
- Prior Medical Assistant experience, preferred
- Experience mentoring and training MAs in clinical skills.
- Complex case management, population health or case management experience
- Triage experience
- Experience educating, monitoring, and providing health coaching for high-risk members
- Proficiency in electronic medical records (EHR) and scheduling platforms.
- Active BLS certification or ability to obtain within 30 days of hire.
- Strong organizational, communication, and customer service skills.
- Willingness and ability to travel weekly between assigned clinics
- Personal cell phone use required for internal communications (Don’t worry, your number will not be shared externally, we use Twilio for external communication).
