Upstate Caring Partners is seeking a detail-oriented and proactive Incident Investigator to support incident management practices across our agency. This role ensures compliance with regulatory standards from OPWDD, OMH, SED, DOH, OCFS, OASAS, and other service programs. The Investigator will play a key role in conducting investigations, presenting orientation components, identifying training needs, and recommending program improvements. This position also supports internal audits, participates in agency activities, and fosters positive relationships with internal and external stakeholders.

 

Core Responsibilities 

  • Conduct and complete investigations in accordance with agency and regulatory standards.
  • Ensure timely and appropriate notifications based on incident classification.
  • Respond to incident reports and provide guidance aligned with policy and compliance.
  • Maintain and update internal and external incident databases (IRMA, NIMRS).
  • Serve as liaison between the Incident Management Department and the Incident Review Committee.
  • Identify and coordinate training needs across the agency.
  • Present orientation components to new employees, including alternate sessions.
  • Attend mandatory training and maintain investigator certifications.
  • Support internal audits and corrective action plans (CAPs).
  • Promote positive communication and collaboration with staff and regulatory entities.
  • Participate in agency committees, meetings, and other related activities.
  • Assist in assimilating new employees into the agency’s culture and mission.
  • Ensure compliance with all relevant regulations and standards.

 

Qualifications 

  • Bachelor’s degree preferred; Associate’s degree with relevant experience accepted.
  • Minimum of 2 years’ experience in a human services field.
  • Must possess a valid NYS Driver’s License.
