As a Primary Care Provider at PACE North, you will:
- Work as part of a multidisciplinary team and collaborate with other professionals to meet the emotional, physical, social, and medical needs of participants
- Deliver care in multiple settings:
- On-site clinic
- Participant homes
- Partner skilled nursing facilities
- Participate in a rotating after-hours on-call schedule to support the on-call nurse
- Build strong relationships with participants and their families through education and support
- Practice in a flexible care model that adapts to participant needs rather than rigid systems
Requirements
We’re looking for a compassionate and collaborative healthcare professional with the following:

- Commitment to interdisciplinary decision-making that prioritizes:

- Respect for self-determination
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Current and valid Michigan license as a Nurse Practitioner or Physician Assistant
- Strong clinical leadership skills
- Excellent interpersonal and communication skills
- Effective problem-solving abilities
- A collaborative mindset with a focus on shared decision-making
- Experience in primary care or geriatric care (preferred)
- An inspirational, motivational, and creative approach aligned with the PACE model of care
