 WestBridge is looking for a full-time LPN to join our brand-new partial hospitalization (PHP) program!


The Licensed Practical Nurse (LPN) plays a vital role in the interdisciplinary treatment team of the Partial Hospitalization Program (PHP), serving adults with serious mental illness and co-occurring substance use disorders. The LPN provides compassionate, recovery-oriented nursing care in a structured, therapeutic environment that emphasizes safety, medication management, health education, and support for individuals working toward stabilization and improved functioning.

As a member of the Medical team, the Nurse is expected to be able to effectively represent and communicate the WestBridge culture of being authentic, team-oriented, and participant-centered.


ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Perform vital signs, general health and psychiatric screenings, focused nursing assessments under RN/APRN/physician supervision.
  • Assist participants in managing psychiatric distress, withdrawal symptoms, cravings, and report and escalate concerns to the RN/APRN/physician as needed.
  • Supervise self-administration of prescribed medications, monitor for side effects, and document according to policy and regulatory requirements.
  • Provide supportive interventions during crises using trauma-informed and de-escalation techniques.
  • Co-facilitates group interventions on selected health issues as requested by APRN/psychiatrist/Medical Director
  • Communicate changes in participant condition promptly to the RN/APRN/psychiatrist, and to the multidisciplinary treatment team.
  • Participate to multidisciplinary treatment team meetings and to the medical team meetings as required
  • Support daily therapeutic programming by reinforcing coping skills, self-care, and healthy routines.
  • Assist in coordinating care with external providers, pharmacies, laboratories and family/support networks as appropriate.
  • Accompanies Participants to medical appointments, exerting coordination and advocacy
  • Oversees all that MD/APRN orders are followed in accordance with WestBridge Policy and Procedures and maintains a safe and orderly medication storage system according to state regulations, nursing guidelines, and WestBridge policies.
  • Educate participants on medication adherence, symptom management, harm reduction, and health promotion.
  • Encourage participation in therapeutic groups and adherence to individualized treatment plans.
  • Model recovery-oriented, stigma-free care that promotes dignity and respect.
  • Maintain timely, accurate, and complete nursing documentation in the electronic health record.
  • Follow PHP, state, and federal standards for safety, infection control, and confidentiality (HIPAA, 42 CFR Part 2).
  • Participate in quality improvement initiatives and regulatory audits as requested
  • Ensure program compliance with local, state and federal regulations. These include but are not limited to confidentiality and medication storage/administration/disposal.
  • Primary affiliation for this role is with the Partial Hospitalization Program, however, duties might include cross-coverage for the other programs (residential, IOP, outpatient) within the allotted hours of duty.
  • Performs other duties as assigned.
Requirements

  • Applicable NH state license. License must be unencumbered, up to date and in good standing.
  • Minimum of two years’ psychiatric nursing experience, as well as medical/surgical nursing.
  • Current BLS/CPR certification required.
  • Active Crisis Prevention Intervention (CPI) Certification required or a plan for timely completion.
  • Skill in client assessment, including withdrawal from substances, change in psychiatric status, physical health and safety and must be able to demonstrate clear understanding of scope of nursing practice.
  • Knowledge of psychiatric medications, addiction treatment principles, and crisis intervention.
  • Skill in group, individual and/or family counseling as appropriate.
  • Knowledge and skill in maintaining care that is stigma-free and trauma-informed.
  • Strong communication, collaboration, and organizational skills.
  • Ability to carry out instruction and accurately document medical information.
  • Ability to work and communicate with others.
  • Demonstrates a high standard of professional conduct, professional boundaries and ethical behavior that will enhance the quality of care and encourage positive interactions among staff, participants and families.
  • Knowledge of state and federal rules and regulations governing confidentiality.
  • Valid drivers’ license and appropriate vehicle insurance coverage of: 100,000 bodily injury per person/300,000 bodily injury per accident/100,000 property damage.
  • Computer skills including but not limited to, Internet access, word processing, · videoconference, electronic medical record, report writing and spreadsheet applications.
  • Successful pre-employment drug screening with negative results required.
  • Regular and reliable attendance is required.

Salary Description
