 Lutheran Homes & Health Services, an Illuminus Community, is looking for a Director of Life Coach Services (SW) to join our Senior Care community. We have been a part of the local community caring for the families of Fond du Lac and the surrounding area since 1936. Conveniently located in the heart of downtown Fond du Lac, Lutheran Homes & Health Services provides assisted living, skilled nursing and short-term rehab services to older adults.

Position Highlights:

  • Full-time, Salaried position
  • $67,000 to $80,000 annualized salary range
  • General Mon-Fri Business Office hour schedule 
  • Required Social Worker Degree/Certification

Our Director of Life Coach Services is responsible for maintaining census goals (for Skilled Nursing Facility & Assisted Living) while managing all referrals to the campus health centers including the following: 


JOB SUMMARY:

An individual professionally trained and employed to; coordinate discharge planning, assist residents and their families in making the adjustment to life in a nursing home, maintain documentation standards, and to advocate for and maintain resident rights.  

ESSENTIAL JOB FUNCTIONS:

  

1. Maintains and actively promotes effective communication with all individuals.

2. Maintains a positive image of the entity in the community keeping in alignment with our mission, vision, and values.

3. Responsible for on-call services, as assigned.

4. Assist new residents and families with the adjustment to community living and the schedules and routines of nursing      home 5. Oversees Resident Care Planning as facilitated by the Life Coach Team. 

6. Coordinates initial staffing/discharge planning with resident, family, and interdisciplinary team.

7. Completes Minimum Data Set and Resident Care Plan related to Social Service.

8. Recommends referrals to appropriate agencies in cases of financial or psychosocial concerns, or desired return      to the community.

9. Acts as discharge coordinator. Provides assistance to other staff, the resident and his/her support system at the time of admission and prior to discharge as under HFS132.

10. Assists the resident in meeting personal needs in the absence of the responsible party.

11. Maintains current social work progress notes and documents interactions as needed.

12. Completes or arranges assistance for residents in completing necessary forms including advance directives.

13. Compiles information about community health and social services available, and keeps a current resource file on these      services.

14. Manages Quality Assurance responsibilities. 

15. Facilitates an environment that encourages residents and families to share any concerns or questions.

16. Is the Grievance Officer and coordinates and manages the process.

17. Acts as supervisor to Life Coaches and completes performance reviews and supervision.

18. Performs other duties, as assigned.

Requirements

QUALIFICATIONS:

Commitment to quality outcomes and services for all individuals.

Ability to exercise independent judgment and make sound decisions.

Ability to adapt to change.

EXPERIENCE AND TRAINING:

Wisconsin Social Work License with 4-5 years of related work experience.

Proven success in working cooperatively with others and relate well to the patients, families, staff, churches and the community in general.
